INTRODUCTION: This is a retrospective cross-sectional study, performed with the aim of evaluating the prevalence of autoimmunity in patients with primary immunodeficency. The innate and adaptive immune systems are both affected by a variety of genetic abnormalities known as primary immunodeficiency diseases (PIDs). The number of unique genetic PIDs has grown tremendously as a result of improved detection techniques and reducing laboratory techniques. Patients with PIDs are more susceptible to autoimmune diseases (ADs), which in some cases are the first signs of PIDs, as well as infectious and non-infectious consequences such allergies, cancer, and malignancies. The primary immunodeficiency diseases come in two varieties. Polygenic immunodeficiency, which are characterized by a heterogeneous clinical presentation that can be explained by a complex pathophysiology and which may have a multifactorial etiology, and monogenic immunodeficiency caused by mutations in genes involved in immunological tolerance that increase the tendency to develop autoimmunity, including polyautoimmunity. The high frequency of ADs in PIDs illustrates the complex interactions between the underlying processes of these two diseases. These individuals' development of ADs is likely to be greatly influenced by deficiencies in central and peripheral tolerance, including, respectively, mutations in AIRE and T regulatory cells. In fact, pathology that causes PID frequently affects the Treg/Th17 balance, which may make it easier for a proinflammatory environment to appear, increasing the likelihood that autoimmunity will develop. Molecular mimicry, bystander activation, and the activation of super antigens are additional factors that are thought to play a crucial role in the development of autoimmunity. These many mechanisms are linked to a variety of clinical sub-phenotypes, which makes it difficult to make an accurate diagnosis in clinical settings and, in some circumstances, may postpone the choice of an appropriate pharmacological therapy. METHODS: In this study, medical records from the archive of patients with Primary immunodeficiency and autoimmunity who were admitted to Imam Khomeini Hospital from 2021 to 2022 were used. The variables required for the study were obtained from the records. The data obtained was analysed by the SPSS software and the significance value of < 0.05 was considered. RESULT: This study comes to a conclusion that there is a significant prevalence of 45.8 % of autoimmunity in the primary immunodeficient patient in the IKHC in 2021 to 2022, and especially the patients with CVID and XLA, becasuse the presence of the autoimmunity was more common in these two groups of primary immunodeficiencnt patients.
