مقدمه و هدف: نقص شناختی از علائم متداول بیماری ام¬اس با شیوع تقریبی 40 تا 70 درصد است. سرعت پردازش اطلاعات، توجه و حافظه فعال بیشترین آسیب را در افراد مبتلا دارند. اثرات تحریک مستقیم فراجمجه¬ای (tDCS) بر عوارض مختلف مربوط به ام¬اس از جمله نقص¬های حسی-حرکتی، درد، خستگی و خلق بررسی شده¬اند، اما مطالعات کمی اثرات آن را بر عملکرد شناختی در این افراد بررسی کرده¬اند. بنابرین هدف از این مطالعه بررسی تاثیر tDCS بر توجه، حافظه¬ی فعال و سرعت پردازش اطلاعات در افراد با ام¬اس می¬باشد.روش پژوهش: در این پژوهش مداخله¬اي و یک سوکور، از بین افراد با ام¬اس بستري در بیمارستان رفیده به روش نمونه¬گیري غیر احتمالي در دسترس 36 نفر انتخاب شده و به صورت تصادفي با استفاده از فایل کامپیوتری SAS در سه گروه 12 نفره قرار گرفتند. پس از انجام پیش آزمون¬ها، گروه مداخله تحریک الکتریکي واقعي(20 دقیقه با شدت 2 میلي آمپر) و گروه کنترل یک تحریک الکتریکي ساختگی دریافت کردند؛ اما گروه کنترل دو تحریکی دریافت نکرد. تمرینات شناختی روتین در هر سه گروه اجرا شد. جلسات 30 دقیقه¬ای 4 بار در هفته به مدت 21 روز انجام شد. در نهایت داده¬ها در نرم افزار spss نسخه 26 مورد بررسی قرار گرفت.یافته¬ها: نتایج نشان داد که بین سه گروه در متغیرهای حافظه¬ی فعال و سرعت پردازش اطلاعات تفاوت معناداری دیده شد. متغیر توجه با وجود تغییرات قابل ملاحظه در عملکرد افراد اما نمرات به لحاظ آماری معنادار نبود. نتیجه گیری: نتایج مطالعه نشان داد که تحریک الکتریکي مغز با tDCSهمراه با تمرینات شناختی در مقایسه با تمرینات شناختی به صورت روتین می¬تواند باعث بهبود مهارت¬های شناختی در افراد با ام اس شود و به عنوان یک ابزار درمانی برای بهبود این نقص¬ها قابل استفاده است. واژگان کلیدي: ام¬اس، توجه، حافظه فعال، سرعت پردازش اطلاعات، مشکلات شناختی، تحریک جریان مستقیم فراجمجمه¬ای

Introduction and purpose: Cognitive impairment is one of the most common symptoms of MS, which affects approximately 40% to 70% of people at any time during their disease. Information processing speed, attention, and working memory are the most damaged in people with MS. In recent years, the effects of transcranial direct current stimulation(tDCS) have been investigated on various complications related to MS, including sensory and motor deficits, spasms, pain, fatigue, and rarely cognitive disorders. Also, in psychiatric disorders such as schizophrenia or neurological disorders such as Parkinson's, tDCS has been used successfully in improving cognitive problems. Therefore, this study aims to investigate the effect of tDCS on attention, working memory, and information processing speed in people with MS.Research method: The present study is an interventional type of clinical trial, one-sided blind. The statistical population of this research was people hospitalized with MS. The samples were selected by non-probability sampling and were randomly divided into three groups. The number of samples in all three groups was 12 people (36 people in total). After the pre-tests, the intervention group received real electrical stimulation along with cognitive rehabilitation interventions and the first control group received a sham electrical stimulation along with cognitive rehabilitation interventions, and the second control group received only two cognitive rehabilitation interventions. In each treatment session, the patients in the interventional group received electrical stimulation with an intensity of 2 milliamps for 20 minutes. The total time of each session was 30 minutes, which was held 4 times a week for three weeks. The current research used some outcome measures to measure attention, active memory, and information processing speed, which consists of the listening sequential summation test (PASAT), and four subtests of the CANTAB computer test, including Stocking of Cambridge (SOC), Rapid visual information processing (RVP), Reaction time (RTI) and Paired associate learning (PAL). Findings: The results of the study showed that the difference between the mean scores of working memory measured by PAL and SOC tests and the speed of information processing measured by RTI and PASAT tests was significant for the attention variable, despite significant changes in participant's performance, the scores were not statistically significant.Conclusion: The results of the study showed that using tDCS along with cognitive rehabilitation interventions compared to routine cognitive rehabilitation exercises can improve attention, working memory, and information processing speed in people with MS and as a therapeutic tool can be used to improve the cognitive defects of these people.Keywords: Multiple Sclerosis, Information processing speed, attention, working memory, transcranial direct current stimulation, Cognitive rehabilitation.
