یکی از رایجترین ناهنجاریهای تکاملی میباشند که از نظر سببشناسی چندعاملی بوده و عوامل ژنتیکی و محیطی در ایجاد آن موثرند .شواهد نشان میدهد آنزیم گلوتـاتیون اس-ترانسفراز (GST) در متابولیسم و سمزدایی دامنه وسیعی از سوبستراهای شیمیایی و سمی شرکت میکنند .با توجه به ناگزیر بودن تماس مادر با مواد سمی و شیمیایی محیطی با توانایی بالقوه تراتوژن در دوران بارداری و نقش عملکردی ثابت شده آنزیمهای GST در ارتباط با متابولیسم و دفع این مواد شیمیایی و سمی، مطالعهی ما به بررسی ارتباط پلیمورفیسمهای شایع خانواده ژنیGST ، شامل(GSTP :۱(rs ۱۶۹۵) و حذف هموزیگوت ژنهایGSTM ۱ وGSTT) ، و بروز شکافهای لب و کام پرداخته است .علاوهبراین با توجه به اطلاعات موجود بیماران با شکاف لب و کام شرکتکننده در این مطالعه ارتباط عوامل محیطی و اجتماعی( همچون مصرف فولیکاسید در مادران و سیگار در والدین و نیز ازدواج فامیلی) و بروز شکاف لب و کام نیز در نظر گرفته شدهاست .روش کار :در این مطالعه ۱۰۵فرد مبتلا به ناهنجاری شکاف لب با یا بدون کام غیرسندرمی و ۲۰۰ فرد سالم بهعنوان گروه شاهد شرکت داده شدند .تعیین ژنوتایپ حذف هموزیگوت ژنهایGSTM ۱ وGSTT ۱ توسط روش PCR چندگانه و تعیین ژنوتیپGSTP ۱(rs ۱۶۹۵) بوسیله PCR RFLP صورت گرفت .نتایج :در مورد پلیمورفیسمGSP ۱(rs ۱۶۹۵)، نتایج نشاندهنده توزیع متفاوت معنادار آلل ورینت (G) در بیماران نسبت به نمونههای کنترل سالم بودند = (P ۰۲.۰ .)به این ترتیب فرکانس ژنوتیپ GG نیز توزیع متفاوت آشکاری را در بین نمونههای بیمار نسبت به کنترل نشان داد = (P ۰۰۴.۰ = OR ,۶.۴ .)نکته جالب اینکه این توزیع نامتفاوت ژنوتیپ GG تفاوت بارز جنسیتی نیز داشت .بدین صورت که جمعیت بیماران مونث واجد ژنوتیپ هموزیگوت قابل توجهی بودند = (P ۰۱.۰ = OR ,۹۱.۷ .)در مورد پلیمورفیسمهای حذفی، ژنوتیپ هموزیگوتGSTM۱ یا در اصطلاحnull ، همراهی قابل توجهی با فنوتیپ شکاف لب و کام نشان داد = (P ۰۴.۰ = OR ,۷.۱ .)در صورتی که در مورد حذف هموزیگوت ژنGSTT ۱ ارتباط معناداری در بین دو گروه مورد بررسی یافت نشد .علاوهبراین، نتایج این مطالعه مصرف مکمل فولیکاسید توسط مادر را عامل محافظتکننده و ازدواج فامیلی و مصرف سیگار توسط والدین را بعنوان ریسک فاکتورهای ناهنجاری شکاف لب و کام نشان دادند .بحث :نتایج مطالعه حاضر بیانگر نقش آنزیم GST در تکامل قبل از تولد میباشد .اینکه چگونه این تعامل باعث بروز شکافهای دهانی می-شوند، نیاز به بررسی بیشتر دارد .آنچه مسلم است ریشه این نقش در مکانیسم بیماریزایی را باید در کیفیت متابولیسم مواد بیرونی یا درونی شیمیایی و یا متابولیتهای آنها جستجو کرد .نکته مهم این است که مصرف مکمل فولیکاسید توسط مادر واجد نقش محافظتکننده بسزایی در رابطه با ناهنجاری شکاف لب و کام غیرسندرمی می باشد و بالعکس مصرف سیگار تسریع کننده بروز فنوتیپ میباشد که میتواند مورد بهره برداری مشاوره قبل از بارداری قرار گرفته تا ریسک بروز ناهنجاری شکاف لب و کام غیرسندرمی به حداقل برسد .واژههای کلیدی :پلیمورفیسم،GST ، شکاف دهانی- صورتی، شکاف لب با یا بدون کام غیرسندرمی، اسیدفولیک

Cleft lip and cleft palate, which can also occur together as cleft lip and palate (CL/P), are congenital orofacial deformity caused by abnormal facial development during gestation. Cleft lip and cleft palate are among the most common congenital malformations. CL/P has a multifactorial etiology, comprising both genetic and environmental factors. It was confirmed that the glutathione S-transferase (GST) enzyme is critical for metabolism and detoxification of numerous substrates and environmental pollutants. Due to unavoidable contact of pregnant women with chemical substances and teratogens and the approved functional role of GST enzyme in detoxification and chemical substrates metabolism, in this study the influence of GST gene polymorphisms including GSTP1 (rs1695) and homozygotic deletions in GSTM1 and GSTT1 genes on cleft lip and palate was investigated. In this study, other descriptive information from participants such as folic acid consumption in mother, smoking of parents and consanguineous marriage were analyzed as well. Materials and methods: One-hundred and five individuals with non-syndromic CL/CP as the case group and 200 matched normal persons as the control group were recruited in the study to analyze the selected polymorphisms. To identify homozygote deletion in GSTM1 and GSTT1 genes, multiplex PCR was carried out. Genotype determination of GSTP1 (rs1695) was performed using PCR-RFLP method. Results: It was determined that in the variant state of GSTP1 polymorphism (rs1695), there is a significant different in allele frequency in the case group comparing to control group (P = 0.02). The GG genotype distribution had a major difference between cases and controls (P = 0.004, OR = 4.6). Interestingly, this genotype was sex related and mostly was expressed in female (P = 0.01, OR = 7.91). The homozygote deletion genotype in GMST1 (null mutation), had significant correlation with CL/P (P = 0.04, OR = 1.7). However, homozygote deletion in GSTT1 did not appreciably alter the risk of CL/P. Moreover, this study revealed that the folic acid consumption in mother have a considerable protective role in preventing CL/P. Consanguineous marriage and smoking parents were observed as risk factors for occurrence of non-syndromic cleft lip/palate. Discussion: The results emphasize that GST enzyme is associated with fetal developments but further analysis is required to figure out the exact mechanism of this interaction which leads to cleft lip and palate. Also, there was an evidence, that folic acid consumption in mother has a significant effect in preventing non- syndromic cleft lip and palate. In contrast, smoking and consanguineous marriage accelerates the expression of this defect. Genetics consulting for prenatal diagnostic may have a valuable role in minimizing the risk of non-syndromic cleft lip and palate.
