افتادن یکی از ریسک فاکتورهای مربوط به سلامت سالمندان محسوب می شود .در دوران سالمندی به دلیل کاهش اطلاعات حسی- حرکتی، کنترل راه رفتن سختصتر شده و برای جلوگیری از بی ثباتی حین راه رفتن، نیاز به توجه داشتن افزایش میصیابد .پژوهش حاضر با هدف تعیین اثرکانون توجه درونی و بیرونی برفعالیت الکترومیوگرافی عضلات اندام تحتانی سالمندان با و بدون سابقه افتادن صورت گرفت .پژوهش حاضر نیمه تجربی است .تعداد ده نفر از سالمندان با سابقه افتادن از بین کانون بازنشستگان به صورت در دسترس انتخاب و با ده نفر از سالمندان بدون سابقه افتادن از نظر سن، وزن و قد همتاسازی گردید .با استفاده از دستگاه الکترومیوگرافی مدل+USB۲ و الکترودهای سطحی ژله ای فعالیت الکتریکی عضلات راست رانی، دوسر رانی و ساقی قدامی سمت راست و چپ بدن هنگام راه رفتن عادی، راه رفتن با توجه درونی و بیرونی اندازه گیری شد .در کانون توجه درونی توجه به اندام خود و در کانون توجه بیرونی توجه بر محیط بیرونی) تصویر در فاصله ۱۰ متری (هنگام راه رفتن مد نظر بود .داده های بدست آمده با استفاده از نرم افزارSPSS.۲۳ و آنالیز واریانس با اندازه های مکرر در سطح معنی داری ۰۵/۰ مورد تحلیل قرار گرفت .نتایج نشان داد که شدت فعالیت همسان سازی شده عضله ی BFL سالمندان با سابقه افتادن بیشتر از سالمندان بدون سابقه افتادن در هنگام راه رفتن عادی است .همچنین شدت فعالیت عضلات (TAL,BFL,BFR) سالمندان با سابقه افتادن بیشتر از سالمندان بدون سابقه افتادن در هنگام راه رفتن با توجه بیرونی است .در سایر عضلات اندام تحتانی (TAL,TAR,BFR,RFL,RFR) شدت فعالیت همسان سازی شده متفاوتی هنگام راه رفتن با توجه بیرونی بین دو گروه تفاوتی وجود نداشت .همچنین نتایج نشان داد شدت فعالیت عضلات (TAL,BFL) سالمندان با سابقه افتادن بیشتر از سالمندان بدون سابقه افتادن در هنگام راه رفتن با توجه درونی است .در سایر عضلات اندام تحتانی (TAR,BFR,RFL,RFR) شدت فعالیت همسان سازی شده متفاوتی هنگام راه رفتن با توجه درونی بین دو گروه وجود نداشت .همچنین نتایج نشان داد، بدون در نظر گرفتن اثر سایر عوامل بین سه نوع دستورالعمل) راه رفتن عادی، توجه بیرونی و درونی(، کمترین شدت فعالیت عضلانی در حالت راه رفتن عادی و بیشترین نیز در راه رفتن با توجه درونی دیده شد .همچنین نتایح نشان داد که الگوی تغییر شدت فعالیت عضلات مختلف در دستورالعمل های مختلف مشابه بود و بین عامل نوع دستورالعمل و عضله تاثیر متقابل معنی داری مشاهده نگردید .بین سه عامل گروه، دستورالعمل و عضله تاثیر متقابل معنی داری وجود دارد .بر اساس یافته های تحقیق شدت فعالیت همسان سازی شده عضلات در سالمندان دارای سابقه افتادن متفاوت از سالمندان فاقد سابقه افتادن است .مطالعات تکمیلی نیاز است تا یافته در بستر کاربردی و در بخش های بالینی و فیزیوتراپی استفاده گردد

Falling is one of the risk factors related to the health of the elderly. In old age, gait control becomes more difficult due to a decrease in sensory-motor information, and to prevent instability while walking, it is important to focus of attention. The aim of this study was to determine the effect of focus of attention (Internal, External) on the electromyography of lower limb's muscles during walking in elderly subjects with and without falling risk Methods: The present study is quasi-experimental. Ten elderly people with a history of falling were selected from among the retirees and were matched with non-falling elderly with regarding age, weight, height by non-random sampling. Ten elderly people without a history of age in terms of age, weight and height. Using USB2 + electromyography device and jelly surface electrodes, the electrical activity of the right thigh, biceps and anterior buttocks of the right and left sides of the body during normal gait, external and internal focus of attention were measured. In the internal focus, attention to one's limbs and in the external focus, attention to the external environment (image at a distance of 10 meters) was considered when gaiting. The obtained data were analyzed using SPSS.23 software and repeated measures analysis of variance at a significance level of 0.05. Result: The results showed that the intensity of BFL assimilated muscle activity was higher in the elderly with a history of falls than in the elderly without a history of normal walking. Also, the intensity of muscle activity (TAL, BFL, BFR) of the elderly with a history of falling is higher than the elderly without a history of falling while walking due to external attention. In other lower limb muscles (TAL, TAR, BFR, RFL, RFR) there was no difference in the intensity of matched activity when walking with external attention between the two groups. The results also showed that the intensity of muscle activity (TAL, BFL) in the elderly with a history of falling is higher than the elderly without a history of falling when walking with internal attention. In other lower limb muscles (TAR, BFR, RFL, RFR) there was no different intensity of matched activity when walking due to internal attention between the two groups. The results also showed, Regardless of the effect of other factors between the three types of feedback (normal walking, external and internal attention), the lowest intensity of muscle activity was seen in normal walking mode and the highest in internal attention walking. The results also showed that the pattern of change in the intensity of activity of different muscles in different feedback conditions was similar and no significant interaction was observed between the type of feedback and muscle. There is a significant interaction between the three factors of group, feedback and muscle. According to the research findings, the intensity of assimilated muscle activity in the elderly with a history of falls is different from the elderly without a history of falls. Further studies are needed to apply the findings in the applied context and in clinical and physiotherapy departments
