کبد، مرکز متابولیک حیاتی، همواره مستعد انواع بیماریهایی میباشد که در مرحله نهایی منجر به فیبروز، سیروز، نارسایی حاد کبدی، نارسایی مزمن کبدی و حتی سرطان میشوند. دارورسانی بهینه و اختصاصی در انواع بیماریها، هپاتکتومی، شانتگذاری و سایر مداخلات جراحی جهت کاهش آسیب کبد، پیوند، نگهداری بهینه و احیای مجدد عضو اهدایی بر درک کامل پرفیوژن و انتقال جرم در کبد تکیه دارند. هدف از این پژوهش شبیهسازی دینامیک سیالات محاسباتی (CFD) جریان خون و توزیع زمانی - مکانی دارو در کبد سالم جهت ارزیابی، درک و شناخت عمیقتر پارامترهای همودینامیکی کبد و انتقال دارو با هدف درمان موثرتر کبد میباشد. در این تحقیق هندسههای پارانشیم کبد و عروق ورید پورتال (PV)، شریان کبدی (HA) و ورید کبدی (HV) از تصاویر اسکن CT شکمی یک فرد سالم، تقسیم و بازسازی شدند. مدلها به صورت کوپل شده با المان سهبعدی مثلثی شبکهبندی شدند و مقادیر فشار نوسانی به عنوان شرط مرزی در عروق ورید پورتال و شریان کبدی اعمال شد. تزریق دارو از طریق ورید پورتال انجام گرفت و شرط مرزی آزاد برای خروجی ورید کبدی اعمال شد. پارانشیم کبد محیط متخلخل فرض شد و درنهایت شبیهسازی دینامیک سیالات محاسباتی (CFD) جهت بررسی پرفیوژن خون و توزیع دارو و زمان اشباع انجام گرفت. مقادیر پارامتر سرعت حاصل از شبیهسازی محاسباتی برای عروق HA، PV، HV با مقادیر فیزیولوژیکی ارائه شده در منابع مطابقت داشت. دبی جرمی در PV بالاتر از HA بود که با پرفیوژن بالا از طریق PV مطابقت داشت. در این پژوهش مقدار گرادیان فشار پورتال (PPG) برابر 53/8 میلیمتر جیوه میباشد. با بررسی فارماکوکینتیکی یک رابطه معکوس بین توزیع تاکسول و مقادیر فشار ورودی در HA مشاهده شد بطوریکه توزیع تاکسول در فشار انتهای دیاستول نسبت به فشار سیستول بیشتر بود. اشباع کبد از تاکسول تا 20 ثانیه با سرعت انجام میگیرد و پس از آن روند اشباع با سرعت کمتری ادامه مییابد. لوب راست به دلیل حجم بزرگتر نسبت به لوب چپ تراکم عروقی بالایی داشته و مقادیر سرعت و دبی جرمی در این ناحیه بیشتر است به علت وجود سیستم عروقی پیچیده و چرخش جریان در انشعابات HA و PV و در محل خروجی HV هرگونه تغییر همودینامیکی، پرفشاری و افزایش سرعت ممکن است باعث اختلال عملکرد شود. این تحقیق میتواند یک شمای کلی از توزیع زمانی مکانی برای مواد دارویی و غذایی و همچنین نقاط مستعد آسیب ارائه نماید.

The liver, a vital metabolic center, is always susceptible to various diseases that ultimately lead to fibrosis, cirrhosis, acute liver failure, chronic liver failure, and even cancer. Optimal and specific drug delivery in various diseases, hepatectomy, shunt placement, and other surgical interventions to reduce liver damage, transplantation, optimal preservation and resuscitation of the donated organ rely on a complete understanding of perfusion and mass transfer in the liver. The aim of this research is to simulate the computational fluid dynamics of perfusion and the time-spatial distribution of drug in a healthy liver in order to evaluate, understand and deepen the hemodynamic parameters of the liver and drug transport with the aim of more effective liver treatment. In this research, patient-specific geometries of liver parenchyma and HA, PV, and HV vessels of a healthy liver were segmented and reconstructed from abdominal CT scan images. Mesh was generated on the coupled models with unstructured tetrahedral elements, and transient pressure values were applied as a boundary condition in the portal vein and hepatic artery. Taxol injection was done through portal vein and free boundary condition applied for hepatic vein outlet. The liver parenchyma was assumed to be a porous medium, and finally CFD simulation was performed to investigate blood perfusion, drug distribution and saturation time. The velocity parameter values obtained from the computational simulation for HA, PV, HV vessels were consistent with the physiological values. Mass flow rate was higher in PV than HA, which was consistent with high perfusion through PV. PPG was 8.53 mmHg. By evaluation the pharmacokinetics, an inverse relationship between Taxol distribution and inlet pressure values in HA was observed, so that Taxol distribution was higher at end-diastolic pressure than at peak systolic pressure. The saturation of the liver from Taxol was done quickly until 20 seconds, and after that the saturation process continued at a slower speed. The right lobe has a high vascular density due to its larger volume than the left lobe and the values of velocity and mass flow rate are higher in this part. Due to the complex vascular system and rotational flow in the HA and PV branches and at the HV outlet, any hemodynamic change, high pressure and increased velocity cause thrombosis and dysfunction. This research provided a point of view of the spatio-temporal distribution of medicinal and food substances as well as damage-prone points.
