سیستمهای مکانیکی حمایتکننده قلب یکی از راه حل های مهم در مراحل مختلف درمان نارسایی های قلب و عروق محسوب میشوند .امروزه بخش عظیمی از تحقیقات در زمینه مهندسی پزشکی به ابداع و توسعه این سیستمها اختصاص داده شده است .شناخت عملکرد همودینامیکی این سیستم ها بخش موثری در طراحی و توسعه تجهیزات مرتبط دارد .این پژوهش به شناخت عملکرد همودینامیکی نوع خاصی از پمپهای تقویت دیاستولی اختصاص یافته است .این پژوهش از دو قسمت اصلی تشکیل شده است .در بخش اول به معرفی یک مدل جدید شرط مرزی( مدل لوله متخلخل) برای خروجی جریان خون در شریانهای بزرگ پرداخته شده است .به کمک مدلسازی ریاضی و شبیهسازی عددی توانایی این مدل در شبیه سازی هر سه ویژگی اصلی پاییندست جریان( مقاومت، کامپلیانس و انعکاس موج) نشان داده شده است .با استفاده از شبیهسازی های عددی متنوع اعتبار سنجی این مدل مورد ارزیابی قرار گرفته است و نتایج نشان دادهاند که مدل لوله متخلخل میتواند برای شبیهسازی های جریان خون در عروق بزرگ بسیار مناسب باشد .همچنین با مقایسه این مدل با مدل لوله الاستیک، نشان داده شده است که مدل لوله متخلل عملکرد بهتری دارد و در عین حال استفاده از آن راحتتر است .بعلاوه نشان داده شده است که به راحتی میتوان مقدار انعکاس امواج از پایین دست را با تغییر مقدار نفوذپذیری بخش متخلخل کنترل کرد .در بخش دوم عملکرد همودینامیکی دو مدل پمپ تقویت دیاستولی جدید( دستگاهPulse - Cو دستگاه KCV) را با استفاده از شبیهسازی عددی مورد بررسی و مقایسه قرار دادهایم .در کلیه شبیهسازیها از مدل شرط مرزی جدید ارائه شده در بخش اول استفاده شده است .برهمکنش دیواره رگ با خون مدنظر قرار گرفته و از روش حل یکپارچه جهت حل معادلات سیال و جامد استفاده شده است .در این شبیهسازیها از هندسه نزدیک به واقعیت استخراج شده از تصاویر پزشکی یک شخص سالم استفاده شده است .نتایج بدست آمده نشان میدهند که دستگاهPulse - Cعلیرغم اندازه کوچکتر بالون، عملکرد بهتری در زمینه خونرسانی به قلب و اندامهای محیطی داشته است .افزایش جریان کرونری برای دستگاهPulse - Cدر حدود ۲۱ درصد و برای دستگاه KCV در حدود ۵.۱۲ درصد ارزیابی شده است .افزایش فشار کرونری نیز به ترتیب به مقدار ۲۴ و ۹ میلیمتر جیوه برای دو دستگاهPulse - Cو KCV بدست آمده است .همچنین نشان داده شده است هر دو دستگاه کاهش پسبار تقریبا یکسانی را بوجود میآورند .با مطالعه و بررسی میدانهای فشار و سرعت بدست آمده مشاهده شده است که گردابههای نسبتا بزرگی در زمان شروع انقباض بالون برای مدت کوتاهی بوجود میآیند که ممکن است مشکلاتی از قبیل لخته شدن خون را بوجود آورند اما برای دستگاهPulse - Cمیدانهای فشار و سرعت در وضعیت نرمالی ارزیابی شدهاند .همچنین بررسی تنش در دیواره رگ نشان داده است که برای دستگاهPulse - Cامکان شروع یا تشدید فرایندهای ریمادلینگ به علت بالا بودن میزان تنش وجود دارد .در نهایت چنین نتیجه گیری شده است که قرار گرفتن بالون در نزدیکی ریشه آئورت اثرات پاد ضربانی بهتری دارد و عملکردPulse - Cدر کمک به بهبود وضعیت قلب مناسب تر است اما مشاهده برخی عوامل نامناسب نشان داده اند که تایید کارایی هر دو دستگاه در درمان بلند مدت نیازمند مطالعات و ملاحظات بیشتر است .

Mechanical circulatory support systems are one of the most important solutions in the various stages of treatment of cardiovascular failure. Today, a large part of medical engineering research is devoted to the invention and development of these systems. Understanding the hemodynamic performance of these systems is an effective key in the design and development of such equipment. This study is dedicated to understanding the hemodynamic performance of a specific type of diastolic augmentation device. This study consists of two main parts and these two parts are complementary to each other. In the first part, a new boundary condition model (porous tube model) for blood flow outflow in large arteries is introduced. Through mathematical modeling and numerical simulation, the ability of this model to simulate all three main downstream characteristics (resistance, compliance, and wave reflection) has been demonstrated. The validation of this model has been evaluated using various numerical simulations and the results have shown that the porous tube model is suitable for simulations of blood flow in large vessels. Also, by comparing this model with the elastic tube model, it is shown that the porous tube model has better performance while it is easier to be implemented in numerical simulations. In addition, it is shown that the amount of reflection of waves from downstream can be easily controlled by changing the amount of permeability of the porous part. In the second part, we have compared the hemodynamic performance of two new diastolic augmentation devices (C-Pulse and KCV) by numerical simulations. In all simulations, the new boundary model presented in the first part is used. The interaction of the vessel wall with blood is considered and the direct solution method is used to solve the fluid and solid equations simultaneously. in these simulations, a patient-specific geometry extracted from medical images of a healthy person is used. The results show that the C-Pulse device, despite its smaller balloon size, performed better in delivering blood to the heart and peripheral organs. The coronary circulation increase is estimated to be about 21 for the C-Pulse device and about 12.5 for the KCV device. Increased coronary pressure of 24 and 9 mm Hg was obtained for C-Pulse and KCV, respectively. Both devices have also been shown to produce an almost identical after-load reduction. By studying the pressure and velocity fields, it is observed that relatively large vortices appear for a short time at the beginning of the contraction of the KCV balloon, which may cause problems such as blood clotting. the pressure and velocity fields are found to be normal for the C-Pulse. Also, the study of tension in the vessel wall has shown that it is possible to start or intensify remodeling processes due to the high amount of stress in the case of the C-Pulse. Finally, it is concluded that the placement of the balloon near the aortic root has a better counter-pulsation effect and the C-Pulse performs better in improving the condition of the heart, but the observation of some irregular flow patterns has shown that the effectiveness of both devices in Long-term treatment requires more studies.
