هدف :یکی از مشکلات متداول در بعضی از کودکان کاربرکاشت حلزونی، عدم پیشرفت مطلوب در کسب مهارتهای شنوایی علیرغم برنامهریزی صحیح پردازشگر گفتار، نداشتن معلولیتهای جانبی و شرکت مستمر در برنامههای توانبخشی شنوایی است .بررسی دقیقتر مسیرها و مراکز پردازش شنوایی در سطح پایین ساقه مغز، از طریق اندازهگیری آستانه و زمان واکنش رفلکس الکتریکی رکابی(ESRT) ، و بررسی ارتباط آن با مهارتهای درک شنوایی میتواند به تصمیمگیری درباره انتخاب روش توانبخشی مناسبتر کمک کند .روش :در این مطالعه ۲۰ کودک کاربرکاشت حلزونی در محدوده سنی ۳ تا ۷ سال انتخاب شدند .حداقل دوسال از زمان دریافت پروتز کاشت حلزونی در همه کودکان گذشته بود و حداقل یک سال از برنامه رسمی توانبخشی شنوایی-کلامی استفاده کرده بودند .زمان واکنش و آستانه رفلکس الکتریکی رکابی (ESRT) برای ۳ الکترود در محدوده راسی، میانی و قاعدهای حلزون اندازهگیری شد .برای بررسی میزان پیشرفت مهارت های شنوایی از ابزار طبقه بندی عملکرد درک شنوایی (CAP) استفاده شد .همبستگی بین نمره بهدست آمده در CAP با زمان واکنش و آستانه ESRT در هریک از سه الکترود با استفاده از آزمون اسپیرمن بررسی شد .نتایج :نتیجه این مطالعه، نشاندهند همبستگی معنادار و منفی بین زمان واکنش ESRT و نمرات بهدست آمده در CAP در الکترودهای راسی بود( ۰/۰۵ >pو ۰/۵- .) =r این همبستگی در الکترودهای میانی و قاعدهای مشاهده نشد .همبستگی معناداری بین آستانه ESRT و نمرات بهدست آمده در CAP در هیچ یک از سه الکترود مشاهده نشد( ۰/۰۵ .)

Objective: One of the common concerns in some cochlear implanted children is the lack of the desired progress in listening skills despite proper speech processor programming, lack of additional disabilities and continuous participation in Auditory-Verbal therapy. Closer examination of the pathways and centers of hearing processing at the lower end of the brain stem by measuring Threshold and reaction time of ESRT, and investigation of its relation to auditory perception skills can provide significant information about choosing the appropriate rehabilitation method. Methodology: In this study, 20 cochlear implant children, aged 3 to 7, were selected. All of them had implanted the cochlear at least two years ago. They all had participated for at least one year in the auditory- verbal rehabilitation program. The threshold and reaction time of ESRT were measured for 3 electrodes in the apex, middle and basal areas. The CAP test score was obtained to assess the progress of auditory performance. Then correlation between CAP test score and ESRT reaction time was investigated in each of the three electrodes. Results: The results showed a significant negative correlation between the ESRT reaction time and the CAP test points in the apex electrodes (r = -0.5, p<0.05). This correlation was not observed in the center and electrodes. There was not any significant correlation between ESRT threshold in 3 electrodes and CAP scores (p>0.05). Conclusion: It seems that there is a significant correlation between the shorter reaction time of ESRT in the apex electrode and the higher scores in the test for the acquisition of hearing skills in children. It is likely that the usefulness of cochlear implantation could be predicted by examining the ESRT reaction time to choose a better rehabilitation program.
