Introduction: Vestibular, vision and somatosensory systems are three major sources to control posture. On the other hand, postural control is the result of neuromuscular and cognitive cooperation and interaction. So, with the goal of investigating balance strategy and pattern, this research was designed using dual task paradigm accompanied by visual and somatosensory input manipulation. Method: In this study, 20 healthy young olds, 20 healthy young adults and 20 young adults with knee pain were recruited. Force plate was used to assess postural control (mean velocity and area variables) and auditory Stroop task (error ratio and reaction time variables) was used with factorial pattern to create dual task condition. Repeated measure analyze was used to analyze the results statistically. Results: In double leg standing condition, area and mean velocity were statistically different (p< 0/05). During one leg standing on dominant leg, young old group were different in area variable (p< 0/05) but this difference was not observed for mean velocity variable (p> 0/05). Mean velocity was significantly different between young olds and young adults with knee pain (p< 0/05). In standing on non-dominant limb, aging adults and young adults with knee pain were significantly different in area variable (p< 0/05) but mean velocity did not show such significancy(p> 0/05). Discussion and conclusion: Pattern change had occurred in double leg standing in comparison of young old with young adults when performing dual task. But this self-organizing behavior in the present chaos has been started from safer condition such as double leg standing. Some young olds were unable to stand one leg showing the stepping strategy instead of ankle strategy. Elderly balance balance pattern were similar to young healthy individuals and in some other conditions showed this similarity to young participants with knee pain. But variability level in elderly was less than young paticipants in general. It seems that elderly tend to reduce the sway variability in order to maintain balance contrary to the youth. Young olds with slower reaction time and less fast muscles do not let their centre of gravity and mass to go further than their support area.so their balance would be stiffer. Dual task have a damping effect but the freezing phenomenon would decrease the degree of freedom. Absence of visual input increased sway in all three groups noticeably. Key words: balance, dual task, visual input, somatosensory input, healthy young old, knee pain
