SUMMARY OR ABSTRACT
Text of Note
Introduction: Successful ageing is a multidimensional and interdisciplinary concept. Despite the great volume of research on successful ageing, there is no consensus on its meaning and measuring. Thus, the aim of this dissertation is conceptualization, instrument development, and validation of successful ageing in the context of Iran. Data and Method: In this study, we used mixed method approach (quantitative and qualitative data) and particularly IDCV meta-framework to development and validation of the successful ageing instrument. IDCV method including 10 sequential phases that started by conceptualization successful ageing and its dimensions using directed content analysis approach (integrative review and a qualitative study of older adults on perceptions successful ageing). Then, item pool designed and evaluated by an expert panel to make content validity and face validity. Revised instrument applied for 600 older adults in Tehran city via a survey to measure construct validity and convergent validity of the instrument. And also, we did crossover analysis (QUAN & QUAL) to validate the instrument. Finally, we explore the dimensions and correlates of successful ageing among older adults. In this process, we used directed content analysis and MAXQDA for qualitative data analysis, and for quantitative data analysis, statistical techniques such as explanatory data analysis, canonical correlation, ANOVA, and MCA in SPSS software was applied. Results: Exploring of successful aging concept and dimensions through integrative review and directed content analysis of qualitative data that gathered from 64 participants indicated 16 sub-categories and 6 main categories of successful ageing, respectively, including social well-being, psychological well-being, physical health, spirituality and transcends, financial security, and elder-friendly social context and environment. Based on findings from this phase (particularly qualitative data), initial items pool including 187 items designed and its content and face validities evaluated by 12 experts. Based on expert panel evaluation initial instrument revised and reduced to 85 items. The revised initial instrument was filled by participants in qualitative study and instrument reduced to 81 items, then to validate initial instrument we made crossover analysis, that means we quantize (0, 1) the sub and main categories in qualitative data for every participant via the inter-respondent matrix, then we test canonical correlation between this set of data and the set of the score of older adults in the initial instrument of SA, the results showed significantly and high correlation between Quan and Qual data. Therefore, the revised instrument with 81 items was prepared to field test. After conducting a survey and gathering data from 600 older adults, explanatory factor analysis was applied to test structural (factorial) validly of the instrument. The results indicated the items of instrument reduced to 54 items and loaded into seven factors that named psychological well-being, social support, financial and environmental security, spirituality, physical and mental health, functional health, and health-related behaviour. In addition structural validity, the instrument has a good degree of convergent validity (is the measure sufficiently consistent with the SWLS instrument), and known group validity (sensitive to differences and similarities in various groups; e.g. different social classes and different health status). Also, the SA instrument had a high degree of reliability coefficients; internal consistency (0.93 for Cronbach alpha) and test-retest stability (0.97 for ICC). In addition, cross-over analysis indicated a significant correlation between Quan and qual findings of the instrument and as result good validity of the SA instrument. Finally, we compare the seven dimensions of successful ageing and explore their socio-demographic correlates. The results showed the older adults had the highest score in the spirituality and functional health dimensions, and conversely, the lowest score in the psychological well-being and financial security dimensions. Also, less than 10 percent of older adults succeeded above average score in the all of SA dimensions. Moreover, the results of the multivariate analysis (MCA) revealed successful ageing and its dimensions were prominently explained by education, socio-economic status, health status and absence of chronic diseases, living arrangement, and neighborhood SES. Conclusion: Social policy in the area of successful ageing should consider all seven dimensions of successful ageing in the individual levels (such as physical and mental health), buffer/compensation (social support), and social context (financial and environmental security).
