هدف :بررسی و مقایسة اثر دشواری شناختی بر اجرای واکنش تعادلی در برابر اغتشاش بین بیماران مبتلا به کمردرد غیر اختصاصی تکرار شونده و افراد سالم پیشینه تحقیق :مطالعات متعددی به بررسی ویژگی های کنترل پوسچر در بیماران مبتلا به کمردرد پرداخته اند .اما نقش بالقوة فرآیند پردازش اطلاعات) توجه (در کنترل پوسچر این بیماران هنوز ناشناخته است .روش انجام کار :در پژوهش حاضر از روش تکلیف دوگانه جهت بررسی تأثیر تکلیف شناختی از نوع استروپ شنیداری بر پاسخ پوسچرال بیماران با سابقة کمردرد غیراختصاصی تکرار شونده در مقایسه با افراد سالم استفاده شد .از افراد خواسته می شد روی صفحة نیروی متحرکی که امکان جابه جایی خطی سطح اتکا را فراهم می کرد قرار بگیرند .صفحة نیرو تحت شرایط مختلف از دشواری شناختی جا به جا می شد .سطوح دشواری شناختی شامل (1) بدون تکلیف شناختی و (2) انجام استروپ شنیداری بود .سطوح دشواری تکلیف پوسچرال نیز از طریق تغییر اندازة اغتشاش وارده دستکاری شد .از اطلاعات جا به جایی مرکز فشار جهت محاسبه پارامترهای تعادلی استفاده شد .یافته ها :آنالیز واریانس نشان داد به دنبال افزایش دشواری شناختی، گروه افراد مبتلا به کمردرد رفتار متفاوتی نسبت به گروه سالم داشت .هم چنین تأثیر افزوده شدن بار شناختی بر پارامترهای واکنش تعادلی) سرعت و دامنه (بر حسب شدت اغتشاش در افراد مبتلا به کمردرد متفاوت بود .اعمال بار شناختی در شدت متوسط، منجر به افزایش دامنه و سرعت واکنش تعادلی در گروه افراد مبتلا به کمردرد شد ولی در شدت اغتشاش بزرگ باعث کاهش سرعت شد .نتیجه :با توجه به نتایج پژوهش حاضر، به نظر می رسد کمردرد از جمله عوامل تأثیر گذار بر ارتباط متقابل کنترل پوسچر و شناخت باشد .کلمات کلیدی :کمردرد غیر اختصاصی، تعادل، پوسچروگرافی دینامیک، توجه

Objectives: To compare effect of cognitive difficulty on balance recovery reactions between subjects with and without recurrent nonspecific low back pain (LBP) Background: Numerous studies have investigated postural control characteristics in LBP patients, but the potential contribution of cognitive processes in postural control of these patients is still unclear.Methods: A dual task paradigm was used to determine effect of concurrent auditory stroop task on balance recovery reactions to postural perturbations in a group of patients with recurrent non specific LBP compared to a healthy, matched group. Subjects were instructed to stand on a moveable platform with each foot placed on a separate force plate. Platform was translated under varying conditions of attentional load: (a) no secondary task, (b) auditory stroop task. Levels of postural difficulty were also manipulated via perturbation size. Center of pressure displacement data was derived and used for calculation of postural parameters.Results: Mixed model of analysis of variance showed that LBP sufferers had different postural behaviors under performance of an attention demanding task. Besides, effect of attention of postural parameters of LBP group was different based on perturbation size. Performance of auditory stroop task caused increase in amplitude and velocity of postural response at medium size of perturbation, but decrease in velocity at large size.Conclusions: Rather than neurological and aging conditions as the factors modulating the dual-tasking effects, this study suggests that LBP as a musculoskeletal disorder may be considered as another factor modulating posture-cognition interaction.Key words: Non specific LBP, Balance, Dynamic Posturography, Attention
