Despite recent advances in treatments, the management of breast cancer is still challenging and controversial. Researches for organic-based alternatives has been motivated by the negative effects of traditional medicines like radiation and chemotherapy. The flavonoid hesperetin has been shown to alter gene expression and so, slow the growth of breast cancer cells. This investigation sought to examine the effects of hesperetin (HSP) at three different doses (200, 400, and 600 M/mL) on the expression of the MLH1 and MSH2 genes in breast cancer (BT-549) and normal (MCF-10a) cell lines by real-time polymerase chain reaction. The cytotoxic and anti-malignant efficacy of hesperetin was tested at three different dosages using the MTT assay, real-time polymerase chain reaction (PCR), and flow cytometry in breast cancer (BT-549) and normal cells (MCF-10a). The cytotoxic effects of hesperetin were shown in both normal and breast cancer cells, with the latter experiencing a more significant response. The IC50 values for BT-549 and MCF-10a, respectively, were 279.2 and 855.4 µg/mL after 48 hours incubation with therapy. The MLH1 and MSH2 genes were upregulated in both cell lines at the highest HSP doses. HSP had a concentration-dependent effect on MLH1 gene expression, with a smaller effect in normal (MCF-10) compared to breast cancer (BT-549) cell lines. Because of their ability to correct genetic mutations, the MLH1 and MSH2 genes are considered anti-tumor genes. Higher concentrations of Hesperetin had a greater effect on MLH1 than lower ones (600 M/mL vs 400 M/mL, 200 M/mL, and control) because hesperetin increased gene expression at higher concentrations. In contrast to the MCF-10a cell line, the BT-549 cell line seemed to have a concentration-dependent response to hesperetin's influence on MSH2 gene expression. Hesperetin increased MSH2 gene expression in the BT-549 cell line at doses of 400 and 600 M/mL compared to the control, while it had no effect on MSH2 gene expression in the MCF-10a cell line at any concentration. Flow cytometric analysis demonstrated that the concentration of hesperetin used to induce cell death and early apoptosis was dose-dependent. Breast cancer patients may benefit from hesperetin because of its ability to induce apoptosis. Furthermore, it has a significant effect on the malignant cell line rather than normal cells. This research suggests that there is a promising perspective for the development of novel breast cancer treatments based on natural herbal compounds like HSP.
