Abstract:Bacterial antibiotic resistance is a clinical concern, because it worsens the infectious illness problem. Multidrug resistant bacteria (MDRs) concerns, particularly nosocomial infections are gaining increasing attention since new medicines are not easily accessible. The current study was aimed to identification of the most prevalent bacterial species in AL- Najaf city that are resistant to antibiotics, determination of antibiotics that are sensitive or resistant to Salmonella Typhi, determination of resistance gene in S. Typhi in AL- Najaf city, and find out if S. Typhi isolates are resistant to multiple antimicrobial agents (MDR). A prospective study was conducted during November 2021 till April 2022. 382 patients suspected to have typhoid fever, who presented for evaluation to the emergency department or inpatient floors of the adult and pediatric teaching hospitals , Al-Hakeem General Hospital and Al-Zahraa Teaching Hospital. Eligible patients were those with an illness of more than 2 days duration that included fever >38 C, who were clinically suspected by the examining physician to have typhoid fever. Blood were collected by using syringes and needles and inoculated in blood culture bottles. Phenotype and molecular study were done for diagnosis antibiotic resistance, also sequencing for whole genome was done. The results showed 38 samples were diagnosed S. Typhi where 150 samples were showed no growth, and194 samples were showed other bacteria species. All samples were resistance for ( Cefazolin, Cefoxitin , Amikacin and Gentamicin) while all samples were sensitive for ( Piperacillin/ Tazobactam, Ertapenem, Imipenem, Ciprofloxacin, Livofloxacin, Tigecycline, Nitrofurantoin and Trimethopirm/ Sulfamehoxaz). 36 samples were resistant for amoxicillin. 35 samples were resistant for ampicillin, Ceftriaxone and Cefepime. DNA was extracted from 38 samples that were diagnosed as (S. Typhi ) amplified by Polymerase chain reaction (PCR) by using specific primers.The antibiotic resistance genes that were targeted in present study were bla- TEM gene with single band (766 bp). bla GES gene with single band (692 bp), bla- OXA gene with single band (928 bp) and bla-CTX-M with single band (550 bp). The %age of bacteria containing genes (bla-CTX-M , bla- TEM, bla- OXA and bla GES) out of the 38 samples was 7.89%, and these genes were repeated in three samples. 11 samples were positive for (bla- CTX-M, bla-TEM and bla GES). 2 samples were positive for (CTX, bla- OXA and bla GES), 1 samples were positive for (bla-CTX-M, bla-TEM, and bla- OXA), 6 samples were positive for (bla-CTX-M , and bla-TEM),8 samples were positive for (bla-CTX-M and bla GES), 2 samples were positive for bla-CTX-M, and bla- OXA , 1 samples were positive for bla- TEM and bla GES, 1 samples were positive for bla-CTX-M and 1 samples were positive for bla-TEM. The highest %age was 28.94% for bacteria containing the following genes bla-CTX-M, bla-TEM and bla GES. The study concluded S. Typhi bacteria were resistant to antibiotics (Cefazolin, Cefoxitin, Amikacin and Gentamicin). While (Piperacillin/ Tazobactam, Ertapenem, Imipenem, Ciprofloxacin, Livofloxacin, Tigecycline, Nitrofurantoin and Trimethopirm/ Sulfamehoxaz) were effective against S. Typhi bacteria. The study indicate a significant increase in the antibiotic resistance of S. Typhi , compared with studies conducted in neighboring countries. The bla-CTX-M gene was the most prevalent gene among the samples, which is responsible for the resistance of the cephalosporin family. The most dangerous gene is that the cephalosporin familyrepresents the first line of treatment used by doctors to combat S. Typhi in Iraq.

مقاومت باکتری نسبت به آنتی بیوتیکی از اهمیت بالینی برخوردار است، زیرا باعث وخیم تر شدن بیماری عفونی می شود. مسئله مقاومت باکتری نسبت به چند دارو (MDRs) بهویژه در عفونتهای بیمارستانی توجه فزایندهای را به خود جلب کرده است زیرا داروهای جدید به راحتی قابل دسترسی نیستند. مطالعه حاضر با هدف شناسایی شایع ترین گونه های باکتریایی مقاوم نسبت به آنتی بیوتیک در شهر نجف، تعیین آنتی بیوتیک های حساس یا مقاوم نسبت به سالمونلا تیفی، تعیین ژن مقاومت در سالمونلا تیفی در شهر نجف و دریافتن اینکه آیا ایزوله های سالمونلا تیفی در برابر عوامل ضد میکروبی متعدد مقاوم هستند یا خیر، انجام گرفته است. یک مطالعه آیندهنگر از نوامبر 2021 تا آوریل 2022 انجام شد. 382 بیمار مشکوک به حصبه بودند که برای ارزیابی به بخش اورژانس یا بخش بستری در بیمارستان های آموزشی بزرگسالان و کودکان، بیمارستان عمومی الحکیم و بیمارستان آموزشی الزهرا مراجعه کردند. بیماران واجد شرایط کسانی بودند که بیش از 2 روز بیمار بودند و >38 °C تب داشتند و پزشک معاینه کننده از نظر بالینی آنها را مشکوک به داشتن تب حصبه می دانست. با استفاده از سرنگ و سوزن خون جمع آوری شده و در بطری های کشت خون تلقیح شد. مطالعه فنوتیپ و مولکولی برای تشخیص مقاومت آنتی بیوتیکی و همچنین تعیین توالی برای کل ژنوم انجام شد. طبق نتایج 38 نمونه با سالمونلا تیفی تشخیص داده شدند و 150 نمونه هیچ رشدی را نشان ندادند و 194 نمونه گونه های باکتریایی دیگر را نشان دادند. همه نمونه ها نسبت به (سفازولین، سفوکسیتین، آمیکاسین و جنتامایسین) مقاوم بودند و همه نمونه ها نسبت به (پیپراسیلین/تازوباکتام، ارتاپنم، ایمی پنم، سیپروفلوکساسین، لیووفلوکساسین، تیگسیکلین، نیتروفورانتوئین و تری متوپیرم/ سولفامهوکساز) حساس بودند. 36 نمونه نسبت به آموکسی سیلین مقاوم بودند. 35 نمونه به آمپی سیلین، سفتریاکسون و سفپیم مقاوم بودند. از 38 نمونه که با (سالمونلا تیفی) تشخيص داده شده بودند DNA گرفته شد و با واکنش زنجيرهاي پليمراز (PCR) با استفاده از پرايمرهاي اختصاصي تقویت شد. ژن های مقاومت آنتی بیوتیکی که در مطالعه حاضر مورد هدف قرار گرفتند، ژن bla-TEM با تک باند (766 جفت باز)، ژن bla GES با تک باند (692 جفت باز)، ژن bla-OXA با تک باند (928 جفت باز) و bla-CTX-M با تک باند (550 جفت باز) بودند. درصد سن باکتری های حاوی ژن (bla-CTX-M، bla-TEM، bla-OXA و bla GES) در 38 نمونه 89/7 درصد بود که این ژن ها در سه نمونه تکرار شدند. 11 نمونه برای (bla-CTX-M، bla-TEM و bla GES) مثبت بود. 2 نمونه برای (CTX، bla-OXA و bla GES)، 1 نمونه برای (bla-CTX-M، bla-TEM، و bla-OXA)، 6 نمونه برای (bla-CTX-M و bla-TEM)، 8 نمونه برای (bla-CTX-M و bla GES)، 2 نمونه برای (bla-CTX-M و bla-OXA)، 1 نمونه برای (bla-TEM و bla GES)، 1 نمونه برای (bla-CTX-M) و 1 نمونه برای (bla-TEM) مثبت بود. بالاترین درصد سنی برای باکتری های حاوی ژن های bla-CTX-M، bla-TEM و bla GES 94/28 درصد بود. در این مطالعه به این نتیجه رسیدیم که باکتری سالمونلا تیفی نسبت به آنتی بیوتیک های (سفازولین، سفوکسیتین، آمیکاسین و جنتامایسین) مقاوم هستند. در حالی که (پیپراسیلین/ تازوباکتام، ارتاپنم، ایمی پنم، سیپروفلوکساسین، لیووفلوکساسین، تیگسیکلین، نیتروفورانتوئین و تری متوپیرم/ سولفامهوکاز) در برابر باکتری سالمونلا تیفی موثر بودند. این مطالعه نشان دهنده افزایش قابل توجه مقاومت آنتی بیوتیکی سالمونلا تیفی در مقایسه با مطالعات انجام شده در کشورهای همسایه است. ژن bla-CTX-M شایع ترین ژن در بین نمونه ها بود که عامل مقاومت در خانواده سفالوسپورین¬ها می¬باشد. خطرناک ترین ژن آن ژنی است که خانواده سفالوسپورین ها آن را نشان می دهد و اولین خط درمانی است که برای مبارزه با سالمونلا تیفی توسط پزشکان در عراق مورد استفاده قرار گرفته است.
