Purpose: In recent years, the introduction of remote after‑loading technology such as high‑dose‑rate (HDR) brachytherapy becomes the safest and more precise way of radiation delivery compared to classical low‑dose‑rate (LDR) brachytherapy in the treatment of vaginal cancer. However, the axially symmetric dose distribution of HDR with single channel cylindrical applicator, the physical “dead‑space” and over dose of vaginal mucosa with multichannel applicators, and shielding material heterogeneities and overdose to the unshielded parts are the main challenges of HDR vaginal cancer radiation therapy. Thus, this study aimed to quantitatively evaluate the dose enhancement factor (DEF) produced by high atomic number nanoparticles (NPs) which increases the interaction probability of photons mainly through the photoelectric effect induced in the great number of atoms contained in each nanoparticle. The NPs loaded to the target volume create a local intensification effect on the target tissue that allows imparting the prescribed therapeutic dose using lower fluxes of irradiation and spare the surrounding healthy tissues.Methods and Materials: Measurement of air kerma strength per source activity were performed at 100 cm far from the source using well type chamber and depending on the recommendations of the AAPM TG-43 formalism report MC simulation was performed using Geant4 code version 11.00 for HDR MicroSelectron 192Ir and HDR Flexi 60Co sources. The simulation was verified by comparing the computed values of the radial dose function, the anisotropy function, and the dose rate constants in a water phantom with previous published data whereas the air kerma strength per source activity in air against the measured value of the well type chamber and previous published data. The optimum NPs were selected through simulation of bismuth (Bi), gold (Au), and platinum (Pt) NPs after verification under the irradiation of 192Ir and 60Co radionuclides. To measure the dose enhancement factor (DEF) and the dosimetric effects of the chosen NPs on the health tissue and OARs (urinary bladder and rectum), at the end simulation of the optimal NPs under the irradiation of 92Ir radionuclide was done after loading into the vaginal tumor.Results: The absorbed dose increased from 5.3% to 20.1% and from 4.2% to 16.5% when the tumor was placed 1 and 2cm away from the radionuclide source, respectively, as the concentration of Au NPs rose from 7 to 30mg/g without significant effect on healthy tissue of the vagina and sparing the OARs (urinary bladder and rectum). The absorbed dose of the urinary bladder and rectum on average was reduced by 3.4% and 4.6%, for 30mg/g concentration, respectively, when the tumor position was 1cm away from the source. Conclusion: Au NPs escalated the absorbed radiation dose within the tumor during 192Ir HDR vaginal brachytherapy without significant effect on the healthy tissue around the tumor and sparing the OARs (urinary bladder and rectum).

هدف: در سالهای اخیر، معرفی فنآوری پس بارگذاری از راه دور مانند براکیتراپی با آهنگ دز بالا (HDR) به ایمنترین و دقیقترین روش پرتودرمانی در مقایسه با براکیتراپی کلاسیک با آهنگ دز پایین (LDR) در درمان سرطان واژن تبدیل شده است. با این حال، توزیع دز متقارن محوری HDR با اپلیکاتور استوانهای تک کاناله، فضای مرده فیزیکی و دز بیش از حد مخاط واژن با اپلیکاتورهای چند کاناله، و شیلدینگ ناهمگنیهای مواد و دز بیش از حد در قسمتهای بدون محافظ، چالشهای اصلی پرتو درمانی سرطان واژن در HDR هستند. بنابراین، این مطالعه با هدف ارزیابی کمی فاکتور افزایش دز (DEF) ناشی از نانوذرات با عدد اتمی بالا (NPs) انجام شد که احتمال برهمکنش فوتونها را عمدتاً از طریق اثر فوتوالکتریک القا شده در تعداد زیادی اتم موجود در هر نانوذره افزایش میدهد. نانوذرات بارگذاری شده در حجم هدف، یک اثر تشدید موضعی روی بافت هدف ایجاد میکنند که اجازه میدهد دز درمانی تجویز شده را با استفاده از شارهای کمتر، تابش داده و بافتهای سالم اطراف را حفظ کند.مواد و روشها: اندازه گیری شدت Air-kerma به روی اکتیویته منبع، در فاصله 100 سانتی متری از منبع با استفاده از well type chamber انجام شد و با توجه به توصیه های گزارشده در پروتکل AAPM TG-43 شبیه سازی MC با استفاده از کد Geant4 نسخه 11.00 برای منابع HDR MicroSelectron 192Ir و HDR Flexi 60 انجام شد. این شبیهسازی با مقایسه مقادیر محاسبه شده تابع دز شعاعی، تابع انیزوتروپی و ثابتهای نرخ دز در یک فانتوم آب با دادههای منتشر شده قبلی با در نظرگرفتن شدت کرمای هوا بر اکتیویته منبع در هوا، در مقایسه با مقدار اندازهگیری شده Well type chamber و داده های منتشر شده قبلی، تأیید شد. نانوذرات بهینه از طریق شبیهسازی نانوذرات بیسموت (Bi)، طلا (Au) و پلاتین (Pt) پس از تأیید با تابش رادیونوکلئیدهای 192Ir و 60Co ، انتخاب شدند. برای اندازهگیری فاکتور افزایش دز (DEF) و اثرات دزیمتری نانوذرات انتخابی بر روی بافت سالم و OARs (مثانه و رکتوم)، در پایان شبیهسازی نانوذرات بهینه تحت تابش رادیونوکلئید 192Irپس از بارگذاری در تومور واژن انجام شد.نتایج: هنگامی که تومور به ترتیب در فاصله 1 و 2 سانتی متری از منبع رادیونوکلئید قرار گرفت، دوز جذب شده از% 3/5 به% 1/20 و از% 2/4 به% 5/16 زمانیکه غلظت نانوذرات طلا از 7 به 30 میلی گرم در گرم رسید، افزایش یافت بدون اینکه تأثیر قابل توجهی بر روی بافت سالم واژن و OAR ها (مثانه و رکتوم) داشته باشد. دز جذب شده مثانه و رکتوم به طور متوسط به ترتیب %4/3 و% 6/4 برای غلظت 30mg/g، زمانی که تومور 1 سانتی متر از منبع فاصله داشت، کاهش یافت.نتیجهگیری: نانوذرات طلا دز تابشی جذب شده داخل تومور را حین براکیتراپی واژینال Ir HDR192 افزایش دادند، بدون اینکه تأثیر قابل توجهی بر بافت سالم اطراف تومورو OARs ( مثانه و راست روده) داشته باشند.
