INTRODUCTION: End stage renal disease (ESRD) is irreversible kidney damage that can be fatal without renal replacement therapy. ESRD is classified as stage 5 of chronic kidney disease (CKD) with an eGFR less than 15 or if dialysis is needed. However, this classification may not apply to children under 2 years old due to their age-adjusted GFR. ESRD is less common in children, but it poses a significant challenge for pediatric nephrologists. The etiologies of ESRD in children vary significantly and mostly include CAKUT, hereditary nephropathies, and glomerular diseases. Renal transplant is the ideal long-term treatment modality for ESRD in children, but dialysis is often required, with peritoneal dialysis being the preferred option in younger children. The outcome of ESRD in children depends on various factors, including economic status, compliance, age, gender, race, underlying diseases, RRT modality, and time duration on RRT.METHOD: This is descriptive, retrospective cross-sectional study. All the patients at ESRD stage of CKD receiving any modality of RRT in Children’s Medical Center, Tehran in years 2020-2022 were considered in this study. The data was collected by medical records and files of the patients and then entered into SPSS26 for analysis.RESULTS: There were total 47 participants in the study. The mean age in the participants was 8.53 years. 66 % were male participants and 34% were female. 63.8 % patients received CAPD and 36.2% received hemodialysis as an initial modality of RRT. Out of all these patients, 12.8% later received renal transplant. The mean duration of dialysis is 2.59 years in both CAPD and hemodialysis patients but there is no significant difference in dialysis duration in between the two groups. The most common underlying diseases are CAKUT including renal dysplasia, ARPKD and PUV. Another common disease is neurogenic bladder. In the underlying diseases too there was no significant difference between CAPD and hemodialysis patients. Overall in all ESRD patients mortality was 40.43%. The frequency of mortality in CAPD patients was higher than hemodialysis ones (51.9 vs. 28.6%) but it was found to be statistically insignificant. CONCLUSION: The most common diseases observed were congenital anomalies of kidney and urinary tract (CAKUT) including renal dysplasia, ARPKD, Posterior Urethral Valves. Neurogenic bladder was another commonly found etiology in these patients. The overall mortality in ESRD patients was 40.43%. There was so significant difference observed in underlying diseases and mortality between the patients receiving CAPD and hemodialysis.

مقدمه: بیماری کلیوی مرحله پایانی (ESRD) آسیب غیرقابل برگشت کلیه است که بدون درمان جایگزین کلیه می تواند کشنده باشد. ESRD به عنوان مرحله 5 بیماری مزمن کلیه (CKD) با eGFR کمتر از 15 یا در صورت نیاز به دیالیز طبقه بندی می شود. با این حال، این طبقه بندی ممکن است برای کودکان زیر 2 سال به دلیل GFR تنظیم شده با سن آنها اعمال نشود. ESRD در کودکان کمتر شایع است، اما چالش مهمی برای نفرولوژیست های کودکان است. علل ESRD در کودکان به طور قابل توجهی متفاوت است و بیشتر شامل CAKUT، نفروپاتی های ارثی و بیماری های گلومرولی است. پیوند کلیه روش درمان طولانی مدت ایده آل برای ESRD در کودکان است، اما دیالیز اغلب مورد نیاز است و دیالیز صفاقی گزینه ارجح در کودکان کوچکتر است. نتیجه ESRD در کودکان به عوامل مختلفی از جمله وضعیت اقتصادی، انطباق، سن، جنسیت، نژاد، بیماری های زمینه ای، روش RRT و مدت زمان RRT بستگی دارد.روش: این مطالعه توصیفی، مقطعی و گذشته نگر است. تمامی بیماران در مرحله ESRD بیماری مزمن کلیه که در سالهای 1391-1399 هر کدام از روشهای RRT را در مرکز طبی کودکان تهران دریافت کرده بودند، در این مطالعه مورد بررسی قرار گرفتند. داده ها توسط پرونده و پرونده های پزشکی بیماران جمع آوری و برای تجزیه و تحلیل وارد نرم افزار SPSS26 شد.نتایج: در مجموع 47 شرکت کننده در این مطالعه حضور داشتند. میانگین سنی شرکت کنندگان 8.53 سال بود. 66 درصد شرکت کنندگان مرد و 34 درصد زن بودند. 63.8 درصد بیماران CAPD و 36.2 درصد همودیالیز را به عنوان روش اولیه RRT دریافت کردند. از مجموع این بیماران، 12.8 درصد بعداً پیوند کلیه دریافت کردند. میانگین طول مدت دیالیز در هر دو گروه CAPD و همودیالیزی 2.59 سال است، اما تفاوت معنی داری در طول مدت دیالیز بین دو گروه وجود ندارد. شایع ترین بیماری های زمینه ای CAKUT از جمله دیسپلازی کلیه، ARPKD و PUV است. یکی دیگر از بیماری های شایع مثانه نوروژنیک است. در بیماری های زمینه ای نیز تفاوت معنی داری بین CAPD و بیماران همودیالیزی وجود نداشت. به طور کلی در تمام بیماران ESRD مرگ و میر 40.43٪ بود. فراوانی مرگ و میر در بیماران CAPD بیشتر از بیماران همودیالیز بود (9/51 در مقابل 6/28 درصد)، اما از نظر آماری ناچیز بود.نتیجه: شایع ترین بیماری های مشاهده شده ناهنجاری های مادرزادی کلیه و مجاری ادراری (CAKUT) شامل دیسپلازی کلیه، ARPKD، دریچه های خلفی مجرای ادراری بود. مثانه نوروژنیک یکی دیگر از علل شایع در این بیماران بود. مرگ و میر کلی در بیماران ESRD 40.43٪ بود. تفاوت معنی داری در بیماری های زمینه ای و مرگ و میر بین بیماران تحت درمان با CAPD و همودیالیز مشاهده شد.
