هدف :عوامل عضلانی ـ اسکلتی و حسی ـ حرکتی بسیاری با گردن درد در ارتباط شناخته شده اند، اما ابهاماتی در خصوص میزان ارتباط و اهمیت این عوامل در بروز گردن درد و پیامدهای بالینی حاصل از آن وجود دارد .شناخت میزان ارتباط و درجه اهمیت هر یک از این عوامل با گردن درد در پیشگیری و ارائه برنامه های درمانی مناسب مؤثر می باشد .هدف از انجام این پژوهش بررسی ارتباط عوامل عضلانی اسکلتی، حسی حرکتی و عملکردی با وقوع گردن درد مزمن غیر اختصاصی و پیامدهای بالینی حاصل از آن می باشد .روش :این مطالعه از نوع مقطعی می باشد .۲۲۰ نفر آزمودنی( ۹۸ فرد سالم و ۱۲۲ بیمار مبتلا به گردن درد مزمن) در این مطالعه شرکت کردند .مجموعا ۷۵ متغیر عضلانی اسکلتی و حسی حرکتی مربوط به نواحی ستون فقرات گردنی، پشتی وکمر به همراه مفاصل شانه و ران و لگن و ارتباطات آنها مورد ارزیابی قرار گرفت .نتایج :در بین عوامل عضلانی- اسکلتی و حسی حرکتی ، قدرت عضلات نواحی مختلف>(P ۰۵/۰) و تعادل ایستا در شرایط ایستادن روی یک پا با چشمان باز>(P ۰۱/۰) و بسته>(P ۰۱/۰) بطور معناداری در بیماران کمتر از افراد سالم بود .وجود نقاط ماشه ای به طور معناداری در عضلات گردن و کتف بیماران بیشتر از افراد سالم بود .بر اساس آنالیز PCA ۱۱ گروه ساخته شد که قدرت عضلات نواحی مختلف گروه اول را تشکیل دادند که واریانس بالایی را نسبت به بقیه گروه ها ی PCA توجیه می کند( درصد واریانس =۳۶/۴۷ .)نتایج حاصل از رگرسیون نیز نشان داد که از مجموع متغیرهای مختلف قدرت عضلات نواحی مختلف=( ۵۹/۰) و نقاط ماشه ای عضله لواتور اسکاپولا=( ۱۹/۳) شانس ابتلا به گردن درد را بیشتر افزایش می دهند .بحث و نتیجه گیری :قدرت عضلات نواحی مختلف ارتباط بالایی با یکدیگر دارند .در بیماران مبتلا به گردن درد مزمن قدرت عضلات نواحی مختلف( گردن، تنه، شانه و ران) نسبت به افراد سالم کمتر می باشد .کاهش قدرت عضلات و وجود نقاط ماشه ای در عضلات گردن شانس ابتلا به گردن درد را افزایش می دهد .کلمات کلیدی :گردن درد مزمن، عوامل عضلانی اسکلتی، عوامل حسی حرکتی، عوامل عملکردی
Objective: Various musculoskeletal and sensorimotor factors have been associated with neck pain. Controversy exists regarding the association and relative importance of some of these factors to neck pain while resolving this ontroversy may make preventive and rehabilitative procedures more efficient. The aim of this study was to investigate the association among musculoskeletal, sensorimotor factors and functional disability and occurrence of chronic nonspecific neck pain. Method: This is a cross sectional study. 220 subjects (98 asymptomatic and 122 neck pain subjects) participated in the study. A total of 75 musculoskeletal and sensorimotor factors of cervical, thoracic and lumbar spine in aaddition to shoulder, hip and pelvic regions were investigated. Results: Among musculoskeletal factors, the muscles strength of multiple regions were significantly lower in patients compared to asymptomatic subjects. Among sensorimotor factors, only static balance in single leg eyes open (P<0.01) and closed conditions (P<0.01) was significantly lower in patients than asymptomatic subjects. Greater number of muscles were affected by trigger points in the cervical and scapular muscles of the patients. The principal component analysis identified 11 components. The muscles strength of different regions constructed the first component that explained the most variance ( variance = 36.47). The regression analysis demonstrated that the first component ( = 0.59) and the presence of trigger points in the levator scapula ( = 3.19) increased the odds ratio of neck pain. Coclusion: There is a high association between muscles strength in differen regions. The patients demonstrated lower muscles strength in the cervical, trunk, shoulder and hip regions.
