مقدمه :اگرچه تاثیر ارتزهایی که بصورت رایج برای تنیس البو تجویز می شوند در چندین مطالعه به اثبات رسیده است، اما تاثیر محدودیت حرکاتی که این عارضه را بدتر می کند تا کنون مورد بررسی قرار نگرفته است .هدف :هدف از مطالعه ی حاضر طراحی و ساخت اسپیلنت جدیدی است که موجب محدودیت در حرکات مچ دست و ساعد می گردد .همچنین در این مطالعه به بررسی تاثیر اسپیلنت جدید بر درد، قدرت گریپ و عملکرد در بیماران مبتلا به تنیس البو، پرداخته می شود .نوع مطالعه :شبه تجربی روش اجرا :در ابتدا دو نمونه اسپیلنت) کوکاپ بلند و اسپیلنت اسپیرال (طراحی و ساخته شد .پس از آزمودن اسپیلنت های ساخته شده بروی 13 فرد سالم، اسپیلنت اسپیرال از نظر ایجاد محدودیت در حرکات چرخشی ساعد، به عنوان ارتز مناسب انتخاب گردید .سپس 15 نفر بیمار مبتلا به تنیس البو وارد مطالعه شدند و متغیرهای درد، گریپ و عملکرد در آن ها به ترتیب توسط روش های مقیاس بصری درد ، داینامومتر جامار و پرسشنامه ی DASH ، قبل و پس از استفاده از ارتز اندازه گیری و ثبت شد .نتایج :نتایج به دست آمده نشان می دهد که اختلاف میانگین درد، قدرت گریپ و عملکرد بیماران قبل و پس از چهار هفته استفاده از اسپیلنت اسپیرال معنادار می باشد .( (P<0/05 نتیجه گیری :بر اساس نتایج به دست آمده از این مطالعه می توان نتیجه گرفت که محدود نمودن حرکات مچ دست توام با حرکات چرخشی ساعد می تواند به میزان زیادی در بهبودی درد، قدرت گریپ و عملکرد بیماران مبتلا به تنیس البو موثر باشد .در حالیکه مطالعه ی دیگری مبنی بر مقایسه محدود نمودن حرکت مچ دست به تنهایی در مقابل محدودیت مچ به همراه محدود کردن ساعد، لازم است .کلمات کلید :تنیس البو، ارتز، درد، عملکرد، قدرت گریپ
Study design: This study is quasi-experimental clinical trial design. Background: Althogh the effectiveness of orthoses that are prescribed for tennis elbow have been reported in several studies, but the effect of limitation of movements that are aggravated this condition had not been investigated. Objective: The aim of this study is designing a new orthosis that provides limitation of wrist and forearm motions and investigation its effect on pain, grip strength and function in patient with tennis elbow. Methods: In the first, two sample splint (long cokup & spiral splint) was designed. after testing orthoses on healthy bodies, spiral splint cause more limitation in rotational movement of the forearm. So spiral splint select as an appropriate orthosis to treatment patient. 15 tennis elbow patients were enrolled in this study. The measure of pain, grip force and function were compared using visual analog scale, jamar dynamometer and DASH questionare respectively, at baseline and 4 weeks after treatment. Results: The spiral splint significantly relieved pain, improved function and grip force of patients after 4 weeks application (P<0.05). Conclusions: This new splint seemed to be effective in relieving pain, improving function and increasing grip force due to limitation of the movement of forearm and wrist of patients. Althogh another study is necessary to compare this new designed splint with a cokup that just limit wrist movement.
