زمینه و هدف :تنوع علتصهای مرگ در جوامع سالمندی بالاست بنابراین برای بررسی و تحلیل زمان تا مرگ افراد در چنین جوامعی میصتوان از مدلصهای مخاطرات رقابتی استفاده کرد .هدف از این مطالعه، شناسایی برخی عوامل موثر بر زمان تا مرگ سالمندان با استفاده از مدلص مفصل مخاطرات رقابتی تحلیل بقا با رویکرد بیزی میصباشد .روش بررسی :در یک مطالعه طولی گذشتهصنگر، اطلاعات مندرج در پرونده 510 سالمند پذیرش شده در مجتمع سالمندان گلابچی کاشان استخراج شد .برای شناسایی عوامل موثر بر زمان تا مرگ سالمندان، تشخیصصهای پزشکی مندرج در پرونده سالمندان مدنظر قرار گرفت، سپس مدل مخاطرات رقابتی با استفاده از تابع مفصل کلایتون تحت رویکرد بیزی بر دادهصها برازش داده شد و فواصل باورمند برآورد گردید .از نرم افزار WinBugs برای تجزیه و تحلیل دادهصها استفاده شد .یافتهصها :در بررسی مدل تک متغیره، بر زمان تا مرگ سالمندان به علت بیماریصهای قلبیصعروقی، عوامل سن در آغاز پذیرش، فشار خون بالا، وجود سابقه بیماری قلبی در فامیل، سابقه سکته قلبی و سابقه سکته مغزی دارای اثری معنادار بودند .درمدل چندگانه، بر زمان تا مرگ سالمندان به علت بیماریصهای قلبیصعروقی، متغیرهای قرار داشتن در گروه سنی بین90 - 75سال در آغاز پذیرش (=((67/3,26/1)=CI95 ; 1/2 HR و سابقه سکته مغزی (=((32/3,28/1)=CI95 ; 2 HR دارای اثری معنادار بودند .نتیجهصگیری :برخی عوامل موثر نظیر سن در آغاز پذیرشص، فشار خون بالا، وجود سابقه بیماری قلبی در فامیل، سابقه سکته قلبی، سابقه سکته مغزی، میزان تحرک و مشکل کلیوی بر زمان تا وقوع مرگ در سالمندان شناسایی شدند، لذا توصیه میصشود در اقدامات درمانی وپیشگیرانه به منظور افزایش زمان بقا برای سالمندان عوامل مذکور نیز مدنظر قرار گیرند .کلید واژهصها :سالمندی، بیماری قلبیصعروقی، مخاطرات رقابتی، تابع مفصل، رویکرد بیزی
Background and Aim Wide verity of causes of death exist in ageing societies, so makes it suitable to apply competing risk models in order to investigate and analyze time todeath in this target population. The aim of this study was to identify factors effective on time to death in the elderly using copula competing risk models of survival analysis with Bayesian approach. Materials and Methods In a retrospective longitudinal study, data were collected from 510 elderlys medical documents in Kashan Golabchi elderly nursing home. To identify effective factors associated with time to death of the elderly, physicians diagnosis of cause of death cited inelderlys medical documents were considered, then competing risk model using Clayton copula function under a Bayesian approach was fitted and credible intervals were estimated. To analyze the data WinBugs software was used. Results In univariate analysis, being inthe age at the beginning of reception, hypertension, family history of cardiovascular diseases, history of myocardial infarction, history of strokewere significant for time to death due to cardiovascular diseases in the elderly. In multivariate analysis,being inthe age group75 to90years at the beginning of the reception(HR=2/1; CI 95=(1/26,3/67)) and history of stroke(HR=2; CI 95=(1/28,3/32)) were significant for time to death due to cardiovascular diseases in the elderly. Conclusion In this study some factors such asage at the beginning of reception, hypertension, family history of cardiovascular diseases, history ofmyocardial infarction, history of stroke, physical activity and renal disease were identified as significant factors associated with time of death of the elderly. Therefore, it is highly recommended that, in therapeuticand preventive actions to increase survival time for the elderly the mentioned factors should be taken into account.
