Background: This research is looking to explore the relationship between insecure attachment and the severity of substance use, mediated by two variables: uncertainty mentalization and emotion dysregulation. This research is based on the theory of mentalization for borderline personality disorder and aims to generalize it to the severity of substance use. According to this theory, insecure attachments, particularly disorganized attachment, are the basis of emotion dysregulation and uncertainty mentalization, which play a significant role in many mental disorders, including borderline personality disorder, which has a high comorbidity with substance use. Methods: Through a cross-sectional method and Convenience sampling design, 185 participants aged from 18 to 25 (22/51±1/87) completed the Reflective Function Questionnaire, Difficulties in Emotion Regulation Scale, Relationship Scales, and Prevalence of Substance Use. Also, Pearson's correlation method and path analysis (multiple regression) using conducted SPSS26 and AMOS24 were to analyze the data. Results: The results of the main model showed that all variables had significant and positive correlations with each other (p < 0.01). The initial model of substance use severity confirmed the effect of disorganized attachment on both the uncertainty mentalization (0.54, p < 0.001) and emotion dysregulation (0.47, p < 0.001) variables. In addition, the effect of uncertainty mentalization on the substance use severity variable was also confirmed (0.51, p < 0.001). In the modified model, the effect of disorganized attachment on the uncertainty mentalization (0.54, p < 0.001) was confirmed. Furthermore, the Sobel test showed that disorganized attachment could indirectly affect substance use severity through the uncertainty mentalization (0.30, p < 0.001). However, emotion dysregulation could not demonstrate its direct and mediating role. Conclusion: As far as we know, for the first time, this research was able to extend the theory of mentalization for a borderline personality disorder to the severity of substance use in Iranian young adults. The findings showed a significant positive correlation between the predictor variables and the criterion variable. Moreover, the uncertainty mentalization played a direct and mediating role in substance use severity and can be important in developing preventive and therapeutic interventions for substance use. Keywords: mentalization; emotion dysregulation; insecure attachment; substance use
