تحریک الکتریکی مغز یک روش توانبخشی رایج است که میتوان آن را به سه صورت تحریک با جریان مستقیم، جریان متناوب و نویز تصادفی در نظر گرفت .در این پایانصنامه، روش تحریک الکتریکی مغز با جریان مستقیم مورد بررسی قرار گرفته است .اعمال این روش بهصورت مختلف سنتی که دو الکترودی است و روشهای مدرن که چند الکترودی است، قابل انجام است .در حالت دو الکترودی یک آند و یک کاتد در محلهای مشخص از سر قرار داده میشوند و جریان الکتریکی مشخص و از پیش تعیینشدهای در مدتزمان معلوم به بیمار اعمال میشود .ثابت شده است که تغییراتی که این جریان و میدان الکتریکی ناشی از آن روی فعالیت نورونهای مغزی میگذارد با گذر زمان باعث بهبود ماندگار برخی فعالیتهای مختل شدهصی مغز میشود .در این پایانصنامه، ابتدا با استفاده از MRI مغزی اخذ شده از بیمار مبتلا به سکتهصی مغزی و اختلال آفازیا به بخشبندی مناسب بافتهای مغزی و ضایعه پرداخته شده است .ضایعه و برخی مشخصات آن با تقریب قابلصقبولی در تصویر به چند روش تفکیک شده است و به دو صورت دو و سهصبعدی به نمایش درآمده است .پنج بافت مغزی عمده شامل مایع مغزی-نخاعی، مادهصی خاکستری و سفید، جمجمه و پوستصسر با مشخصاتی ازجمله حجم، تفکیک شده است .مدل سهبعدی مناسب از روی ماسکهای هر بافت استخراج شده است .پس از طراحی مناسب الکترود تحریک در حالت استاندارد و حالتهای پیشنهادی، این الکترودها بهصورت مناسبی روی پوستصسر قرار گرفتهاند .تمامی بافتهای بخشبندی شده به همراه الکترودها مش-بندی شده و روشهای پیشپردازشی مناسب مانند هموارسازی روی آنها اعمال شده است .سپس، در مرحلهصی مدلسازی محاسباتی جریان الکتریکی با دامنهصی مشخص به الکترود تحریک اعمالشده است .شدت میدان الکتریکی و جهت آن در بافتها و نیز در محل ضایعه بهصورت دقیق با نرمافزار بهدست آمده است .این روند روی MRI مغزی چند بیمار سکتهصی مغزی مبتلا به آفازیا تکرار شده است و نتایج مطالعات روی سه شکل الکترود مستطیلی، دایرهصای و مثلثی با سطح مقطع ثابت، مورد بررسی قرار گرفته است و مشاهده صشده است که این اشکال هندسی تأثیر بالقوهصای بر شدت میدان الکتریکی ایجاد شده در محل ضایعهصی آفازیا ندارند .همچنین، مناسبصترین مونتاژ برای این نوع بیماری در این پایانصنامه در نظر گرفته شده است
Brain electrical stimulation is a common rehabilitation method that can be categorized into three categories: stimulation with direct current, alternating current and random noise.In this study, the first type has been investigated. This method can be implemented traditionally with two electrodes or modern way with multi-electrodes. In the case of two electrodes, an anode and a cathode are placed at the specified points of the head, and a specific and predetermined electric current is applied to the patient within the specified period.It has been shown that changes of current and its electric field have a reasonable effect on brain neuronal activity and can improve some disorders of stroke, such as aphasia. It has a lasting effect on disturbed parts of the brain. In the first step, the magnetic resonance imaging (MRI) of stroke patients with aphasia is acquired. Then, the brain gets segmented into five tissues: scalp, skull, cerebrospinal fluid, white matter and gray matter.The lesion and its characteristics are extracted by several tools. In the second step, the appropriate 3D model is extracted from each mask. In the next, after the proper design of the stimulating electrode in the standard or proposed state, these electrodes are placed on the scalp. Finally, all segmented tissues and electrodes have meshed, and suitable preprocessing techniques such as smoothing have applied to them. In the computational modeling and stimulation stage, an optimal electric current with a specific amplitude has been applied to the stimulated electrodes. The intensity of the electric field and electrical potential in various tissues and also in the lesion are obtained accurately with software. This process repeatedly experimented on the brain MRI of several patients with aphasia stroke and the results of the study on how to detect the lesion, and its different methods, and various forms of electrodes are discussed in the present study. It has been observed that these geometric shapes have not to impact on the intensity of the electric field that created at the aphasia lesion
