پیشزمینه :بیماریها و مشکلات مربوط به دستگاه تولیدمثلی در طیور نسبتا شایع هستند .پرولاپس کلوآک و اویداکت، احتباس تخم و سختزایی، و تروماهای وارده به ناحی پرینه که منجر به ایجاد زخم میشوند، ازجمل این مشکلات هستند .در برخی از گونههای پرندگان مانند غاز و شترمرغ مشکلات دیگری مانند پرولاپس فالوس نیز دیده شده است که میتواند در اثر عواملی مانند تروما، عفونت، یا تغییرات شدید آب و هوایی اتفاق افتد .از دیگر مشکلات نیازمند مداخل جراحی یا تهاجمی در ناحی کلوآک میتوان به سنگهای کلوآک اشاره نمود .در این موارد نخستین اقدام درمانهای حمایتی و تغذیهای است .ولی گاهی این درمانها مؤثر واقع نشده و پرنده نیازمند جراحی تحت بیهوشی میشود .علیرغم مشکلات نسبتا زیادی که در دستگاه تولیدمثلی و قسمتهای خلفی بدن پرنده ایجاد میشوند، تاکنون انجام تزریق نخاعی یا اپیدورال در پرندگان گزارش نشده است .همچنین، نه زمان شروع اثر و نه طول مدت اثر داروهای بیحسی ازجمله لیدوکائین در پرندگان مورد مطالعه قرار نگرفته است بنابراین، در پژوهش حاضر فرض بر این قرار گرفت که تزریق داروی بیحسی در فضای زیر عنکبوتیه یا اپیدورال خلفی جوجههای گوشتی بهعنوان مدلی برای پرندگان مشابه، بیحسی مؤثری را در نواحی خلفی بدن جهت انجام مداخلات جراحی فراهم میکند .هدف این پژوهش بررسی امکانپذیری تزریق در کانال نخاعی جوجههای گوشتی با ارزیابیهای آناتومیک و سپس کاربرد این تکنیک در بالین بود .روش کار :در این پایاننامه از ۳۰ قطعه جوجه مرغ گوشتی دوماه نژاد راس با وزن۳/۰ ۲کیلوگرم استفاده شد .پیش از شروع مطالعه یک هفته به آنها زمان داده شد تا با شرایط محیط جدید سازگار شوند .جوجهها بهصورت کاملا تصادفی به پنج گروه شش قطعهای تقسیم شدند .در گروه اول (F) شناسایی آناتومیک در حیوان بهصورت تازه و بدون تثبیت بافتها صورت گرفت .در گروه دوم (S) پس از تثبیت نمونهها، ارزیابی و شناسایی آناتومیکی بهصورت میکروسکوپی انجام گرفت .در سه گروه بعدی بیحسی نخاعی با داروی لیدوکائین ۲ بدون آدرنالین انجام شد .برای شناسایی مختصات آناتومیکی محل تزریق، در گروه اول (F) پس از آسانکشی به روش خونگیری، ارزیابیهای آناتومیکی بدون تثبیت کردن و بهصورت تازه انجام گرفت و فضای بین نخستین مهر آزاد دمی و سینساکروم بهعنوان بهترین جایگاه برای انجام بیحسی نخاعی انتخاب گردید .در جوجههای گروه دوم (S) پس از خونگیری، قسمت ستون مهرهها از سینساکروم تا انتهای مهرههای دمی جداسازی و تثبیت شد .پس از کلسیمگیری، برشهایی با ضخامت ۵ میکرومتر بهصورت ساژیتال و عرضی تهیه شده و با روش هماتوکسیلین-ائوزین رنگآمیزی گردید .در مقاطع تهیهشده ویژگیهای کانال نخاعی و موقعیت نخاع درون آن، فضای اپیدورال و زیر عنکبوتیهای زیر میکروسکوپ نوری مورد بررسی قرار گرفت .در سه گروه بیحسی، سوزن شمار ۲۳ وارد این فضا شده و بسته به گروه موردنظر، بیحسی نخاعی با داروی لیدوکائین بدون آدرنالین در سه دوز ۲ میلیگرم/کیلوگرم۲)- (L، ۱ میلیگرم/کیلوگرم۱) - (Lو ۵/۰ میلیگرم/کیلوگرم۰.۵)- (L، انجام گرفت .تعداد تنفس و دمای کلوآک پیش از تزریق و نیز هر ده دقیقه یکبار تا زمان از بین رفتن اثر بیحسی اندازهگیری شدند .آزمون بیدردی با استفاده از پنس شستی قبل از تزریق و در زمانهای۱ ،۳ ، ۵ دقیقه و پسازآن هر پنج دقیقه یکبار تا زمان بازگشت واکنش به تحریک انجام شد .فاصل بین تزریق و نخستین عدم واکنش به تحریک دردناک بهعنوان زمان شروع اثر دارو ثبت شد .طول مدت اثر دارو نیز از زمان شروع اثر تا زمان از بین رفتن بیحسی در ناحیه ارزیابی گردید .نتایج :در جوجههای گروه نخست (F) فاصل فضای بین نخستین مهر آزاد دمی و سینساکروم تا انتهای دم دو برابر فاصل بین انتهای دم و خلف عانه بود .در جوجههای گروه دوم (S) فاصل نخاع از دیوار استخوانی کانال نخاعی بهخوبی مشخص بوده و فضای وسیع زیر عنکبوتیهای اجاز تزریق داروی بیحسی در این فضا را میداد .شروع اثر لیدوکائین با دوز ۵/۰ میلیگرم/کیلوگرم۰.۵) - (Lبرابر با۵/۱ ، شروع اثر دوز ۱ میلیگرم/کیلوگرم۱) - (Lبرابر با ۲ و شروع اثر دوز ۲ میلیگرم/کیلوگرم۲) - (Lبرابر با ۳۳/۱ دقیقه بوده و هیچکدام از گروهها از این نظر تفاوت آماری معناداری با یکدیگر نداشتند .(p=۰.۶۰۴) طول مدت اثرL - ۲بهصورت معناداری بیشتر از طول مدت اثرL - ۱و۰.۵ - Lبود .(p=۰.۰۰۲) ولی گروههایL - ۱و۰.۵ - Lتفاوت معناداری با هم نداشتند .دمای کلوآک در گروهL - ۲در زمانهای ۱۰ و ۲۰ دقیقه تفاوت آماری معناداری با دو گروه دیگر داشته و از آنها پایینتر بود .همچنین این تفاوت در طول زمان در هر گروه معنادار بود و با گذشت زمان دما کاهش پیدا میکرد .(p<۰.۰۵) در گروهL - ۲تفاوت معناداری بین دمای دقیق ۲۰ و دقیق ۳۰ وجود نداشت .در گروههای۰.۵ - LوL - ۱تعداد تنفس ۱۰ دقیقه پس از تزریق بهصورت معناداری افزایش یافته بود .در گروهL - ۲تعداد تنفس تا دقیق ۲۰ روند کاهشی داشت هرچند این تفاوت معنادار نبود(p<۰.۰۵) ، سپس تعداد تنفس تا دقیق ۳۰ افزایش داشته که نشانگر از بین رفتن اثر داروی بیحسی است .نتیجهگیری :یافتههای این مطالعه نشان داد که در جوجههای گوشتی انجام بیحسی اپیدورال خلفی مقدور نیست لیکن بیحسی نخاعی بهراحتی در بالین قابل انجام بوده و بنابراین میتواند بهعنوان یکی از گزینههای کمهزینه و نسبتا آسان در مواقع درمان مشکلات مربوط به ناحی خلفی بدن مورد توجه قرار گیرد و جایگزینی برای بیهوشی در این پرندگان باشد
Background: Problems related to the reproductive system are relatively common in birds. Cloaca and oviduct prolapses, egg retention and dystocia, and trauma to the perinea resulted in laceration are among these problems. There are other problems such as phallus prolapse in species like geese and ostriches which could result from trauma, infection or severe alterations in weather. Cloaca stones are another problem requiring surgical or invasive intervention in this area. In these situations, the first attempt is supportive and nutritional treatments. But sometimes these treatments are not effective and surgery must be performed in the bird under general anesthesia. Despite relatively common problems in the reproductive system and caudal areas of birds, spinal or epidural anesthesia were not reported, yet. In addition, neither onset of action nor duration of action of local anesthetics including lidocaine are not evaluated in the birds. In this study, we assumed that injection of local anesthetic in subarachnoid or caudal epidural space of broilers as a model of similar birds can provide effective analgesia in the caudal areas of the body for surgical interventions. The aim of this research was to evaluate the feasibility of spinal injection in broilers through anatomic evaluations and then to the clinical use of this technique.Methods: 30 two-month-old Ross broiler chickens were used in this study. There was a week of acclimatization to the new environment. The chickens randomly divided into five groups of six. Anatomic identification in the fresh cadaver without any fixation of the tissues was done in the first group (F). Microanatomic evaluations were performed in the second group (S) after fixation of the specimens. Spinal anesthesia with 2 lidocaine was induced in the three other groups. For anatomic identification of the site of injection in group F, evaluations were done without fixation in the fresh cadaver after euthanasia by exsanguination, and the interval between synsacrum and first coccygeal vertebra was chosen as the best site for spinal injections. The vertebral column of groups S from synsacrum to the end of the coccygeal vertebrae was isolated and fixed. Sagittal and transverse 5 m sections were made after decalcification and were stained with hematoxylin and eosin. Spinal canal properties and position of the spinal cord, epidural and subarachnoid spaces were examined under light microscope. In three anesthesia groups, a 23 G needle was inserted into this space and depending on the group, spinal anesthesia was induced by lidocaine in doses of 2 mg/kg (L-2), 1 mg/kg (L-1), and 0.5 mg/kg (L-0.5). Respiratory rate and intra cloacal temperature were measured before the injection and then every ten minutes until disappearing of the anesthetic effect. The analgesia was tested using a pair of thumb forceps before injection, and 1, 3, and 5 minutes after injection, and continued in 5-minute intervals till returning the sensation. The time between injection and the first loss of reaction to the painful stimuli was recorded as the onset of action. The duration of action was calculated from onset of action to returning of sensation to the area.Results: The distance between synsacro-coccygeal site and the end of the tail was twice than distance between caudal part of the pubis and the end of the tail in group F. The distance between spinal cord and internal lining of spinal canal was obvious in group S, and extensive subarachnoid space made it possible to inject anesthetic into this space. The onset of action in L-0.5 was 1.5 minutes, L-1 was 2 minutes, and L-2 was 1.33 minutes with no significant differences (p=0.604). Duration of action in L-2 was significantly longer than L-1 and L-0.5. But L-1 and L-0.5 had no significant difference to each other. The cloacal temperature in group L-2 had a significant difference with others in times 10 and 20 minutes and was lower than them. In addition, this difference was significant within each group during the time (p<0.05). There was no significant difference between 20 and 30-minute temperatures. Respiratory rate was increased significantly in L-0.5 and L-1 ten minutes after injection. This rate had a decreasing pattern untill minute 20 in group L-2. However, this difference was not significant (p<0.05), afterward, respiratory rate was increased until minute 30, which is a sign of disappearing of analgesic effect.Conclusion: The results of this study showed that caudal epidural anesthesia is not possible in broiler chickens. But spinal anesthesia easily can be performed, thus can be a non-expensive and relatively easy option when treating problems related to the caudal part of the body, which can be a substitution to general anesthesia
