هدف : این پژوهش با هدف شناسایی نقش تجربه آزارهای جسمی، روانی و جنسی بر میزان لغزش و ماندگاری در درمان نگهدارنده با متادون در بیماران وابسته به مواد افیونی تحت درمان نگهدارنده با متادونانجام گرفته است .روش :این پژوهش با تعداد ۱۲۰ نفر شرکت کننده که از جامعه به شیوه نمونه گیری هدفمند از روی پرونده های موجود در کلینیک های درمان سومصرف مواد بر اساس ملاکهای ورود و خروج انتخاب شدند، انجام شد .برای سنجش تجربیات آزار آزمودنی‌ها از پرسشنامه سوابق آزارهای جنسی روانی و جنسی جزومی و پیترز ۲۰۱۰ ،و برای سنجش ویژگی‌های دموگرافیک آن ها ازپرسشنامه اطلاعات دموگرافیک استفاده گردید و در نهایت اطلاعات حاصل از این پرسشنامه ها با استفاده از روش‌های آمار توصیفی و استنباطی با استفاده از نرم افزار SPSS نسخه ۱۶ مورد بررسی قرار گرفت .نتایج :نتایج نشان داد که سوابق آزارهای جنسی روانی و جنسی اثر معنی داری بر نتایج درمان نگهدارنده با متادون ندارد و صرفا از طریق میزان و شدت تجربیات آزار نمی‌توانیم ماندگاری افراد را در درمان پیش‌بینی کنیم .بحث و نتیجه گیری :یافته های بدست آمده از پژوهش حاضر موید آن است که علت و عوامل مختلفی در خروج از درمان و میزان ماندگاری مراجعین نقش دارند و سابقه آزارهای جسمی و روانشناختی یکی از عواملی است که در شروع مصرف، دوام و عود مصرف مواد تأثیر بسزایی میتواند داشته باشد ،لذا درنظر گرفتن سابقه تروما، ممکن است ماندگاری در درمان نگهدارنده را افزایش و میزان لغزش و عود را کاهش دهد .لذا می‌توان نتیجه گرفت که درنظرگیری و توجه به این فاکتور به طور غیر مستقل به منظور حصول نتایج بهتر درمانی حاز اهمیت می‌باشد .کلید واژه ها :آزار روانی آزار جسمی آزار جنسی درمان نگهدارنده با متادون

Background: Sexual, Emotional, or Physical assault occurs commonly during lifetime of patients seeking substance use disorder (SUD) treatment, and it is important to learn how such histories impact on opioid maintenance treatment outcomes. Objectives: The study was designed to investigate and follow the relationship between history of sexual, emotional, or physical abuse and methadone maintenance treatment (MMT) outcomes among opioid dependent males. Materials and Methods: Study samples included 120 opioid dependent males entering outpatient SUD treatment for the first time, chosen by convenient sampling method during December 2013 to March 2014 who were invited to participate in the questionnaire surveys during the first two months of their intake and have been followed up for eight months on basis of lapse, methadone dosage, type of drug abuse during the lapses and relapse. Data were collected as part of an ongoing SUD treatment at Methadone maintenance treatment centers in Tehran/Iran. Results: Results of the current study indicated no significant relationship between past experiences of sexual, emotional or physical abuse and the outcome of methadone maintenance treatment. In terms of comparison based on qualitative variables, higher educational level (P = 0.005), and marital status (being single) were significantly co-related with retention in MMT. Meanwhile, Pierson co-relational test showed that individuals who reported higher numbers of witnessing abuses were under significantly higher dosage of methadone on their last undertake (p<0.05 & r=0.209), while no significant differences were seen between other groups. Conclusions: Identifying the underlying purpose for a greater use of addiction services by drug-dependent individuals with abuse histories seems crucial, in order to make informed referrals and to provide more efficient services with better long-term outcomes.
