SUMMARY OR ABSTRACT
Text of Note
Introduction:The anorectum plays an important role in regulation of defecation and in maintenance of continence. The most widely used test for anorectal function is anorectal manometry. It measures pressures of the anal sphincter muscles, sensation in the rectum, and neural reflexes that are needed for normal bowel movements. This test is mainly used in evaluation of constipation, fecal incontinence, anal sphincter tone, functional anorectal pain, pelvic floor dyssynergia and diagnosis of Hirschsprung’s Disease. Anorectal manometry (ARM) is essential in children with chronic constipation and functional/structural faecal incontinence (CCFSFI) to completely understand the pathophysiological mechanisms involved to direct care, and to ease the discomfort of the patients' symptoms. This study is a cross sectional study with a sample size of 273 patients , the target population in this study involves Children referring to Children Medical Centre with diagnosis ranging from constipation , Hirschsprung disease and Faecal incontinence from time period April 2010 to April 2020. This Study is designed to obtain the main underlying etiologies and outcomes in children undergoing anorectal manometry. Method:To perform this study , medical records from the archive of patients with Constipation, Hirschsprung disease and Faecal incontinence were checked from the clinical setting of Children medical centre Tehran, from April 2010 to April 2020. The variables required for this study were obtained from the medical records. The Data was analysed by the IBM SPSS software and significance value of <0.05 was considered statistically significant. Results: We analysed 273 patients for this study with complaints of Constipation, Hirschsprung disease and faecal incontinence. Age group ranged from 1 years old to 16 years old with mean age 6.05, The males were 141(51.6%) and females 132(48.4%). Out of 273 patients screened during this study 252 patients had constipation(92.3%), 9 Patients had Hirschsprung disease(3.3%) and 12 patients had faecal incontinence(4.4%) . RAIR was present in 123 patients (45.1%). A significant statistical relationship between residual pressure and RAIR.( P-value= 0.0001) and Percentage of relaxation and RAIR( P value=0.0001). No statistically significant relationship was found between resting pressure and gender (P value=0.18), residual pressure and gender (P value=0.38), percentage of relaxation and Gender (P value=0.94). A weak reverse correlation between age and resting pressure (P-value=0.15) and residual pressure and age was found(P value=0.51). Conclusion: From this study we concluded that there was a significant relationship between anorectal manometry parameters like percentage of relaxation and RAIR, residual pressure and RAIR and at the same time there was no significant relationship found between resting pressure and gender, Residual pressure and gender, Percentage of relaxation and gender.
