پیش زمینه: رابطه بین التهاب و بهبود زخم پیچیده است. اگرچه التهاب ناکافی در زخمهای پوستی، بهبود زخم را به تأخیر میاندازد، التهاب بیشازحد یا مداوم نیز از بهبودی جلوگیری میکند. بنابراین، یافتن مداخلهای که هم اثر درمانی بر روند بهبودی داشته باشد و هم توانایی کشتن میکروبها را داشته باشد، ارزش زیادی دارد. عسل یک پانسمان بیولوژیکی زخم با چندین فعالیت زیستی است که برای تسریع روند بهبودی بهطور هماهنگ عمل میکند. هیدروژل ها شبکههای پلیمری سهبعدی با اتصالات عرضی هستند که به دلیل دارا بودن ساختاری شبیه به ماتریکس خارج سلولی (ECM) و توانایی جذب آب در کاربردهای مختلف پزشکی استفاده میشوند. با در نظر گرفتن خواص عسل در بهبود زخم و اینکه موپیروسین اولین خط درمان آنتیبیوتیکی در درمان زخم است، فرض بر این است که استفاده از هیدروژلِ این ترکیب میتواند باعث تسریع روند بهبود زخم شود.روش کار: در این مطالعه از 18 قطعه موش صحرایی نر استفاده شد. موشهای صحرایی بهصورت تصادفی به سه گروه با تعداد مساوی تقسیم شدند: 1-گروه کنترل منفی: در این گروه زخم ایجاد شده و بدون اضافه کردن هیچ مادهای پانسمان گردید. 2-گروه کنترل مثبت: در این گروه پس از ایجاد زخم از هیدروژل عسل درمانی با فرمولاسیون مشخص و ثبتشده برای پانسمان زخم استفاده شد. 3-گروه تیمار: در این گروه پس از ایجاد زخم از هیدروژل حاوی عسل و موپیروسین برای پانسمان زخم استفاده شد. موشهای صحرایی بیهوش شده و پس از موچینی ناحیه پشتی، حالت گماری ونترال و آمادهسازی آسپتیک ناحیه پشتی حیوان دو زخم تمام ضخامت پوستی در دو طرف ستون مهرهها با فاصله 5/1 سانتیمتر از خط وسط در ناحیه قفسه سینه و بالای کتفها با استفاده از پانچ پوستی استریل 8 میلیمتری ایجاد شد. بسته به گروه در نظر گرفتهشده، صفحات هیدروژل بهتنهایی یا آغشته به موپیروسین و ترکیب عسل و موپیروسین بهصورت قطعات دایرهای شکل با قطر 1 سانتیمتر بریده شده و در زیر زخمهای ایجادشده قرار داده شد. سطح زخم با پانسمان شفاف پوشانده شد. پانسمانها در هر سه گروه در روزهای 3، 7، 10و 12 پس از جراحی تعویض شد. از زخمها در روزهای 0، 3، 7، 10، 12 و 14 پس از جراحی عکسبرداری شد تا سرعت بسته شدن زخم مورد ارزیابی قرار گیرد. در روز 14 پس از جراحی، حیوانات آسانکشی شده و از زخمها بهصورت تمام ضخامت نمونهگیری انجام گرفت. از زخمهای سمت چپ برای ارزیابیهای مولکولی به روش PCR کمی استفاده شد. زخمهای سمت راست برای ارزیابیهای هیستوپاتولوژیک به صورت کیفی مورد استفاده قرار گرفت.دادههای مربوط به بسته شدن زخم و تست PCR با استفاده از روش ANOVA یکطرفه ارزیابی شدند. دادههای هیستوپاتولوژیک توسط روشهای کیفی بررسی شد.نتایج: زخم در هر سه گروه در مدت زمان 14 روز بسته شد، اما تفاوت آماری معناداری در بین سه گروه در هر مقطع زمانی وجود نداشت (p>0.05). براساس نتایج بافت شناسی در گروه هیدروژل موپیروسین+ عسل در مقایسه با گروه کنترل رشته های کلاژن بیشتر، متراکم تر و منظم ترند و ادم ندارند. ضمایم پوست مثل غدد سباسه و فولیکول های مو تراکم بیشتری دارند. همچنین براساس نتایج مولکولی بیان ژن های Cox2 و TNF-α در گروه هیدروژل عسل و هیدروژل موپیروسین عسل نسبت به گروه کنترل به طور معناداری بالاتر بود (p<0.05).اما تفاوت معنی داری در بیان این دو ژن بین گروه هیدروژل حاوی عسل و هیدروژل حاوی علسل و موپیروسین دیده نشد.نتیجهگیری: با توجه به نتایج بدست آمده پانسمان هیدروژل حاوی عسل و موپیروسین دارای خواص بیولوژیکی مفیدی بوده و می تواند در بهبود زخم پوستی کاربرد درمانی داشته باشد.

Background: The relationship between inflammation and wound healing is complex. Although insufficient inflammation in skin wounds delays wound healing, excessive or persistent inflammation also inhibits healing. Therefore, finding an intervention that has both a therapeutic effect on the healing process and the ability to kill microbes is of great value. Honey is a biological wound dressing with several biological activities that work synergistically to accelerate the healing process. Hydrogels are three-dimensional polymer networks with cross-links that are used in various medical applications due to their structure similar to the extracellular matrix (ECM) and their ability to absorb water. Considering the properties of honey in wound healing and that mupirocin is the first line of antibiotic treatment in wound treatment, it is assumed that the use of hydrogel of this compound can accelerate the wound healing process.Methods: 18 pieces of male rats were used in this study. Rats were randomly divided into three groups with equal numbers: 1-Negative control group: In this group, a wound was created and it was dressed without adding any substance. 2-Positive control group: In this group, after wound formation, therapeutic honey hydrogel with a specific and registered formulation was used for dressing the wound. 3-Treatment group: In this group, hydrogel containing honey and mupirocin was used for dressing the wound after creating a wound. The rats were anesthetized and after tweezing the dorsal area, ventral position and aseptic preparation of the animal's dorsal area, two full-thickness skin wounds on both sides of the vertebral column at a distance of 1.5 cm from the midline in the chest area and above the shoulder blades using an 8 mm sterile skin punch was created. Depending on the considered group, hydrogel sheets alone or impregnated with mupirocin and the combination of honey and mupirocin were cut into circular pieces with a diameter of 1 cm and placed under the created wounds. The wound surface was covered with a transparent dressing. Dressings were changed in all three groups on days 3, 7, 10, and 12 after surgery. Wounds were photographed on days 0, 3, 7, 10, 12 and 14 after surgery to evaluate the speed of wound closure. On the 14th day after surgery, the animals were euthanized and samples were taken from the wounds in full thickness. Left wounds were used for molecular evaluations by real-time PCR. The wounds on the right side were used for histopathological evaluations.Results: The wound was closed in all three groups within 14 days, but there was no statistically significant difference among the three groups at any time point (p>0.05). Based on the histological results, in the mupirocin+honey hydrogel group, compared to the control group, the collagen fibers are more, denser and more regular, and they do not have edema. Skin appendages such as sebaceous glands and hair follicles are more dense. Also, based on the molecular results, the expression of Cox2 and TNF-α genes in the honey hydrogel and honey mupirocin hydrogel groups was significantly higher than the control group (p<0.05).Conclusion: According to the obtained results, the hydrogel dressing containing honey and mupirocin has useful biological properties and can be used therapeutically in the healing of skin wounds.
