پیشینه تحقیق: امروزه با افزایش جمعیت سالمندان و افزایش میزان بیماری‌های مزمن، شیوع ناتوانی نیز در جهان و ایران افزایش‌یافته و به‌تبع آن ارائه خدمات توانبخشی با پوشش مناسب نیز از اولویت‌های نظام سلامت قرارگرفته است. ایران نیز مانند بیشتر کشورها با پیوستن به کنوانسیون حقوق افراد دارای ناتوانی، متعهد شد که زمینه را برای دسترسی همگانی و برابر به مراقبت‌های بهداشتی و توانبخشی برای همه افراد دارای ناتوانی فراهم سازد. به همین منظور این مطالعه با هدف ترسیم وضعیت تسهیلات توانبخشی (ساختارها و نیروهای توانبخشی) در مقیاس‌های ملی و زير ملی جهت تولید شواهد موردنیاز برای رسیدن به پوشش همگانی سلامت در افراد دارای ناتوانی انجام شده است.روش: این مطالعه یک مطالعه مقطعی بود که بین ماه¬های می و دسامبر سال 2019 انجام شد. داده‌های تسهیلات توانبخشی اعم از زیرساخت‌ها و نیروی انسانی توانبخشی در سطوح مختلف نظام سلامت در سطح ملی و زیر ملی با استفاده از لیست اصلی خدمات توانبخشی جمع‌آوری و با نرم‌افزار Excel نسخه 2016 تجزیه‌وتحلیل شدند.نتایج: نتایج حاصل از این مطالعه نشان می‌دهد که موجودیت ساختارهای توانبخشی در ایران در چهار سطح متمرکز است که شامل:الف) در سطح بستری 1.1 در یک‌میلیون نفر جمعیت تخت توانبخشی ، ب) در سطح سرپایی به ترتیب فراوانی 42.6 مرکز فیزیوتراپی،16.4 دفتر کار بینایی‌سنجی، 11.7 مراکز روزانه و حرفه‌آموزی توانبخشی 10.5 دفتر کار شنوایی‌شناسی، 8.7 گفتاردرمانی، 8.2 کاردرمانی، 4.5 ارتوپد فنی، 3.8 مطب طب فیزیکی و توانبخشی، 1.7 مرکز جامع توانبخشی در یک‌میلیون نفر، ج) در سطح جامعه‌محور:15.9 واحد توانبخشی مبتنی بر جامعه، 1.9 مرکز توانبخشی در منزل، 1.6 مرکز پرستاری در منزل، 0.3 مرکز مراقبت‌های باليني در منزل در یک‌میلیون نفر جمعیت و د) در سطح اقامتی: 4.11 آسایشگاه در یک‌میلیون نفر جمعیت کشور موجود است. در خصوص موجودیت نیروهای انسانی توانبخشی: 84.02 فیزیوتراپی، 34.8 گفتاردرمانی،32.5 کاردرمانی، 31.21 بینایی‌سنجی،27.93 شنوایی‌شناسی،10.25 ارتوپد فنی و 5.11 متخصص طب فیزیکی در یک‌میلیون نفر جمعیت مشغول به کار هستند.نتیجه‌گیری: در ایران تسهیلات توانبخشی ازنظر تعداد و توزیع متناسب با نیازهای افراد دارای ناتوانی نیست که این امر دسترسی عادلانه به خدمات توانبخشی را به‌عنوان یکی از حقوق انسانی این افراد را با مشکلاتی مواجه کرده است. لذا با توجه به ضرورت‌ها و تعهدات جهانی، ملی و منطقه‌ای در خصوص پوشش گسترده این خدمات توصیه می‌شود در خصوص بازنگری سیاست‌ها با هدف بهبود وضعیت موجودیت و توزیع امکانات توانبخشی اقدامات جدی صورت پذیرد.

Background:Nowadays with the increase in the population of the elderly and the number of chronic diseases, the prevalence of disability has raise. Therefore, Iran, like many other countries has committed to provide universal and equal access to health care and rehabilitation for all people with disabilities by joining to Convention on the Rights of People with Disabilities. So, this study was conducted with the aim of drawing availability of rehabilitation facilities (infrastructure and rehabilitation workforce) on national and sub-national scales to produce the evidence needed to achieve universal health coverage in people with disabilities.Methods:This is a cross-sectional study conducted between May and December 2019. Rehabilitation facilities data including infrastructure and rehabilitation workforce in health system settings on national and subnational scales were collected using Master List Rehabilitation Services and analyzed with Excel version 2016 software.Results:In Iran was available in inpatient: 1.1 bed per million population, outpatient: physiotherapy42.6 optometry 16.4,audiology 10.5, occupation therapy8.2 , speech therapy 8.1, orthotic& prosthetic 4.5، physical medicine & rehabilitation 3.8 offices , Day care center 11.7 and Rehabilitation center 1.3, community base rehabilitation: community base rehabilitation unit15.9 , Rehabilitation Home care2, Rehabilitation Nursing Home Care1.6 and Medical rehabilitation Home Care 0.3, Long term care : residential care 4.1 per million population. In rehabilitation work force: physiotherapist 84. Speech therapist 34.8, occupational therapist 32.5, optometrist 31.2, audiologist 27.9, ortesis protesis 10.3 and physical medicine & rehabilitation 5.1 per million population Conclusion:In Iran, rehabilitation facilities in terms of number and distribution are not commensurate with the needs of people with disabilities. This has made it difficult for people to have fair access to rehabilitation services as a human right. Therefore, considering global, national, and regional requirements and obligations universal coverage of these service, it seems that health systems policymakers should pay special attention to the field of rehabilitation in this regard.Keywords: rehabilitation, facilities, availability, people with a physical disability
