مقدمه :بعد از اجرای سیاست‌های موسسه زدایی بیماران روان‌پزشکی، اهمیت شناخت و اندازه‌گیری مفهوم بار مراقبین خانوادگی بیماران مبتلا به اسکیزوفرنیا برای ارائه‌دهندگان مراقبت‌های سلامتی بیش‌ازپیش آشکار گردید .این مطالعه با هدف طراحی و روان‌سنجی پرسشنامه بار مراقبین خانوادگی بیماران مبتلا به اسکیزوفرنیا انجام شد .روش :مطالعه حاضر یک مطالعه ترکیبی متوالی اکتشافی از نوع ابزارسازی است .در مرحله اول پژوهش، با استفاده از مدل تحلیل مفهوم هیبرید شوارتز، بارکوت و کیم، مفهوم بار مراقبین خانوادگی بیماران مبتلا به اسکیزوفرنیا تبیین گردید .در مرحله نظری از مرور یکپارچه استفاده شد و یافته‌ها با استفاده از روش نافل و ویتمور تحلیل گردیدند و در مرحله کار در عرصه با ۱۶ مشارکت‌کننده در درمانگاه و بیمارستان‌های روان‌پزشکی دولتی شهر تهران مصاحبه‌های غیرساختارمند انجام پذیرفت .یافته‌های این مرحله با استفاده از روش تحلیل محتوای قراردادی تحلیل گردیدند .در طی مرحله تحلیل نهایی، نتایج فاز نظری و کار در عرصه به‌منظور ارائه تعریفی جامع از مفهوم بار مراقبین خانوادگی بیماران مبتلا به اسکیزوفرنیا با یکدیگر ادغام گردیدند .در مرحله دوم پژوهش، جهت طراحی پرسشنامه بار مراقبین خانوادگی از رویکرد ۴ مرحله‌ای والتز استفاده گردید .سپس فرایند اعتباریابی پرسشنامه شامل تعیین روایی و پایایی طی شد .یافته‌ها :بر اساس تحلیل مفهوم، بار مراقبین خانوادگی بیماران مبتلا به اسکیزوفرنیا، مفهومی تغییرپذیر درگذر زمان می‌باشد که مراقبین خانوادگی آن را به‌صورت درک فردی عذاب‌آور تجربه و بیان می‌کنند .این مفهوم دارای ویژگی‌هایی مانند دشواری عهده‌داری مسئولیت‌های متعدد، تنش‌های خانوادگی، انزوای اجتماعی و تحمل هزینه‌های سنگین بیماری می‌باشد .بار مراقبین خانوادگی در بستری از مراقبین آسیب‌پذیر، تنگناهای مالی زندگی، تهدیدهای اجتماعی و فرهنگی، رفتارهای منفی و هیجانی و سخت علاجی بیماری بروز می‌کند و باعث ایجاد سلامتی تهدید شده، مشکلات اجتماعی، رهاسازی بیمار و معنی یافتن زندگی می‌گردد .در مرحله دوم، پرسشنامه بار مراقبین خانوادگی با ۳۲ سؤال در ۵ عامل تنش‌های خانوادگی و اجتماعی، آینده مبهم و نگران‌کننده، شرمساری و رنج، توانایی ازدست‌رفته و تقلایی برای محافظت از بیمار طراحی گردید که درمجموع ۵۲/۱۸۴ درصد واریانس کل را تبیین می‌کردند .پایایی ابزار با استفاده از ضریب آلفای کرونباخ ۰/۹۴ و همبستگی درون خوشه‌ای ۰/۹۲ به دست آمد .نتیجه‌گیری :بر اساس این مطالعه، پرسشنامه روا و پایای بار مراقبین خانوادگی بیماران مبتلا به اسکیزوفرنیا (Family Caregiver Burden Questionnaire: FCBQ) مبتنی بر فرهنگ جامعه ایرانی طراحی گردید .همچنین در این راستا مفهوم بار مراقبین خانوادگی بیماران مبتلابه اسکیزوفرنی مبتنی بر کانتکس جامعه تبیین و تعریف شد .کلمات کلیدی :مراقبین خانوادگی، بار، بیماران مبتلا به اسکیزوفرنیا، مطالعه ترکیبی متوالی اکتشافی، روان‌سنجی

Text of Note

Introduction: After the implementation of deinstitutionalization policies in the mental health system, the importance of recognizing and measuring the family caregiver burden in patients with schizophrenia has become increasingly apparent for health care providers. The purpose of this study was to design and evaluate the psychometric properties of a questionnaire on family caregiver burden for patients with schizophrenia. Method: In a mixed method with an exploratory sequential approach, the concept of family caregiver burden was explored using Schwarz Barcott and Kim's concept analysis hybrid model. In the theoretical stage, the integrated review approach was used and Whittemore and Knafls method was used for data synthesis. The conventional content analysis method was used in the field work phase. During this phase, 16 non-structured interviews were conducted with 16 participants in clinics and psychiatric hospitals in Tehran. During the final analysis, the results of the theoretical and the fieldwork phases were combined to provide a comprehensive definition of the concept of family caregiver burden in patients with schizophrenia. The second phase of the study, Waltz's four-step approach was used to design a family caregiver burden questionnaire. Then, its psychometric process that included validity and reliability, was completed. The Psychometric process of the questionnaire was performed. Results: According to the concept analysis, the family caregiver burden of patients with schizophrenia is a changeable concept over time that family caregivers experience and express it as a personal understanding of torment. Other attributions of this concept are such as undertaking hard multiple responsibilities, family tensions, social isolation, and enduring the heavy costs of illness. The family caregiver burden occurs in the context of vulnerable caregivers, the financial hardships of life, social and cultural threats, negative and emotional behaviors, and the difficult treatment of the disease and causes threatened health, social problems, patient abandonment, and meaningful life. In the second phase, a family caregiver questionnaire was designed with 32 questions in 5 factors that included "family and social tensions", "ambiguous and worrying future", "shame and suffering", "lost the ability" and "struggle to protect the patient" which, overall, covered 65 of the variance of all the factors. The Cronbachs alpha for the entire questionnaire was 0.94 and the interclass correlation coefficient based on test-retest method was 0.92. Conclusion: A valid and reliable Family Caregiver Burden Questionnaire in patients with schizophrenia developed based on the Iranian culture. Also, the concept of family caregiver burden in patients with schizophrenia based on the Iranian context was explained and redefined.
