مقدمه :استئوآرتریت از شایعترین بیماریهای مزمن سالمندان است که مستقیما منجر به مرگ نمیشود ولی از علل مهم ناتوانی میباشد .درمان های دارویی و روش های جراحی معمولا همراه با عوارض جانبی و پرهزینه میباشد(۱ ,۲(۳ .)مطالعه حاضر بمنظور بررسی تاثیر روغن موضعی ناخنک در مقایسه با ژل دیکلوفناک بر علائم بالینی استئوارتریت اولیه زانو در سالمندان به عنوان یک روش بدون عارضه انجام شد .روش کار :مطالعه کارآزمایی بالینی شاهددار دو سو کور با کدIRCT ۲۰۱۶۰۸۲۱۲۹۴۶۱N۱ می باشد .جامعه آماری شامل ۶۲ سالمند با میانگین سن ۷۸، انحراف معیار ۷۹.۷، کمترین سن ۶۳، بیشترین ۹۴ با=mode ۸۱ دارای استئوآرتریت تشخیصی متخصص ارتوپدی( استئوآرتریت ضعیف تا متوسط ،گرید ۱ تا ۳ بر اساسL) - (Kو درد متوسط آزار دهنده بر اساس معیار VAS ،ساکن مراکز نگهداری تهران وارد مطالعه شدند .بعد از تکمیل رضایتنامه و معیارهای ورود و خروج وارد مطالعه شده و اطلاعات دموگرافیک، پرسشنامه شدت ارتروز WOMAC و مقیاس بصری درد پرسیده شد .بر اساس جدول رندومیزاسیون به گروه های مداخله و شاهد تقسیم شده، داده ها در بدو مطالعه، ۲ و ۴ هفته بعد از شروع مداخله جمع آوری شده وارد نرم افزار SPSS ۲۳ شدند و روش های آماری توصیفی و استنباطی استفاده شد .یافته ها:ابتدا با آزمون فیشر سن، تحصیلات، وضعیت تاهل، سواد، اشتغال، مصرف سیگار، فعالیت فیزیکی، بیماری مزمن و مصرف دارو تفاوت معناداری نداشتند .براساس نتایج مطالعه درد( F ۱ , ۵۸ =)۰۷.۱۱۹>p ,۰۱.۰ ، خشکی مفصل( F ۱ , ۵۸ =)۹.۱۹>p ,۰۱.۰ ، عملکرد( F ۲ , ۵۸ =)۴.۲۰>p ,۰۰۱.۰ در هر دو گروه مداخله و شاهد بهبود یافت .شدت بهبودی موارد مداخله بیشتر از کنترل بود .عوارض جانبی خاصی گزارش نشد .نتیجه گیری :روغن موضعی ناخنک در مقایسه با ژل دیکلوفناک بر کاهش علائم بالینی استئوآرتریت ضعیف تا متوسط اولیه زانو( درد، شدت درد، خشکی صبحگاهی و عملکرد) موثر و پیشنهاد میگردد .

Introduction: Osteoarthritis or arthrosis is one of the most prevalent chronic diseases among older adults. It may not cause death among older people; however, it is one of the main causes of disability, social and economical burden in seniors. Pharmacological treatments and surgical procedures usually are costly and may have adverse effects. This study aimed to investigate the effect of topical oil Melilotus Officinalis on clinical sings of primary knee Osteoarthritis versus Diclofenac gel: A double blind study. Material and Methods: This study was a double-blind randomized controlled trial (RCT) approved by Iranian Registry of Clinical Trials (IRCT) number IRCT2016082129461N1. The participants were 62 older people aged 60 years and over (mean=78, standard deviation=7.79, range 63-94 and mode=81) who had clinically diagnosed mild to moderate (Kellegren-Lawrence (K-L) grading scale) knee osteoarthritis, having moderate pain in the knee based on Visual Analogue Scale at least 4 weeks before randomization residing in Kahrizak senior house. All the participants signed the consent form and assessed based on inclusion and exclusion criteria. Demographics variables, WOMAC: Western Ontario and McMaster Universities Osteoarthritis Index, Visual Analogue Scale (VAS) were collected through interviews. Based on a balanced - block randomization the study participants were randomly assigned to one of two groups: the experimental group receiving the intervention that is using topical oil Melilotus Officinalis and a comparison (control) group which received diclofenac gel during the study. The data collected at baseline (week 0), the end of week 2 and week 4. All the data entered SPSS version 23 and analyzed after data management based on the study aims. Results: There was no statistically difference between the intervention and control groups in terms of age, education, marital status, employment, smoking, physical activity, chronic disease and medication. There was statistically improvement in pain, stiffness, function and total score of WOMAC questionnaire in both control and intervention groups, although the effect size was higher in the intervention group. No adverse effect reported for topical oil Melilotus Officinalis during the study. Conclusion: According to the results of this study topical oil Melilotus Officinalis can improve pain, stiffness and function more than diclofenac gel. Therefore, topical oil Melilotus Officinalis can be recommended for decreasing clinical symptoms of mild to moderate osteoarthritis.
