هدف:با توجه به شیوع بالای BPPV و برگشت پذیری حملات و عدم بهبودی فرد علیرغم درمان اولیه در تعداد بالایی از بیماران ،مطالعات مختلفی در این زمینه در سالهای گذشته صورت گرفته است . هدف پژوهش حاضر ،درمان BPPV راجعه بااصلاح کمبود ویتامین D و بررسی تاثیرآن بر مانور درمانی Epley بود . روش بررسی :در یک مطالعه کار آزمایی بالینی ، 81بیمارBPPV (54 بیمار با کمبود ویتامینD و 27بیمار بدون کمبود 40 ( تا 60 ساله که در شش ماه گذشته بیش از یک حمله سرگیجه ای داشتند به صورت سه گروه مساوی ،گروه اول و دوم بیماران BPPV با کمبود ویتامین D وگروه سوم بیماران BPPV بدون کمبود ویتامین D تقسیم شدند .هر سه گروه یک دوره مانوردرمانی Epley به مدت یک ماه و هفته ای یکبار دریافت کردند ودر گروه اول علاوه بر مانور،مکمل ویتامین D به مدت دوماه و هفته ای یک عدد (5000IU)دریافت شد . شدت نیستاگموس تورشنال ،شدت سرگیجه و تعداد دفعات تکرار حمله قبل و بعد از مداخله بررسی شد. برای آنالیز آماری از نرم افزار spss و آزمونهای کروسکال والیس و اندازه های تکراری استفاده شده است . یافته ها:نتایج تحلیل های آماری حاکی از آن است که بعد از دو ماه و سپس شش ماه مداخله شدت نیستاگموس تورشنال و تعداد حملات سرگیجه ای در گروه اول و سوم نسبت به گروه دوم کاهش نشان می دهد و شدت سرگیجه وضعیتی در سه گروه در دوماه بعد کاهش را نشان داد و در شش ماه بررسی در گروه اول و سوم این کاهش حفظ و در گروه دوم مجددا افزایش نشان داد .(P<0/001) نتیجه گیری: در بررسی BPPV های راجعه علاوه بر بررسی های مرسوم پیشنهاد می شود که سطح ویتامین D به عنوان یک عامل بروز در این بیماران مدنظر قرار گیرد . کلید واژه ها: سرگیجه وضعیتی خوش خیم ،کمبود ویتامینD ،نیستاگموس تورشنال ،شدت سرگیجه ،تکرارپذیری حملات

Aim: Regarding the high prevalence of BPPV and recurrence of attacks and uncure of patients with classic treatment ,different studies has been carried out .The aim of current study was treatment of recurrent BPPVand investigation of verification of vitamin D deficiency on EPLEY maneuver. Method: In this clinical trial study ,81 BPPV patients (54 with vitD deficiency,27 without vitD deficiency) 40-60 years old that in 6 month ago had at least one vertigo attack were assigned into three equal groups; group I and II with vit D deficiency and group III without deficiency . all of them received EPLEY maneuver weekly for one month .In group I in addition to EPLEY, received additional supplemental vitamin D at 50.000 IU cholecalciferol weekly for two months to achieve serum 25-OHD 30 ng/ml and the study patients followed for 6 months.Vertigo intensity ,tortional nystagmus, number of vertigo attack befor and after intervention were analysed. Spss software and Kruskal wallis and Repeated Measure and Chisqure were applied. Results: Statistical analysis results showed that after two month and then six month intervention , nystagmus intensity , and number attack decreased in group I and III, and vertigo intensity in three groups decreased in two month and this decreament remained in group I,III and increased in group II again (P<0/001( . Conclusion: We showed that vitamin D evaluation in BPPV patients were important . It seems that verification of vitD deficiency ,decrease recurrence of positional vertigo .
