زمینه :بیماری استروک به علت ایجاد ناتوانی جسمی و گاه غیر قابل برگشت در فرد به عنوان یک مسئله خانوادگی نگریسته می شود .اعضای خانواده بیمار استروک نقش مهمی در توانبخشی و بهبود بیمار سالمند استروک ایفا می نمایند .با توجه به افزایش وقوع استروک در عصر جدید و به ویژه در میان سالمندان، هنوز توصیف واضحی در خصوص فرایند تطابق خانواده با فعالیت های روزمره زندگی سالمند مبتلا به استروک وجود ندارد، لذا این مطالعه با هدف تبیین فرایند تطابق خانواده با فعالیت های روزمره زندگی سالمند مبتلا به استروک انجام شده است .روش پژوهش :این تحقیق کیفی به شیوه نظریه زمینه ای در سال1391 - 1393در شهرهای ساری و بهشهر انجام گرفت .روش نمونه گیری در ابتدا هدفمند و سپس بر اساس داده های بدست آمده و نظریه در حال ظهور از نمونه گیری نظری استفاده شد .انتخاب مشارکت کنندگان تا اشباع نظری ادامه یافت .مشارکت کنندگان در این پژوهش 27 شرکت کننده شامل 19 عضو خانواده دارای سالمند مبتلا به استروک، 2بیمار سالمند مبتلا به استروک و 6 نفر کارشناس از اعضای تیم بهداشتی درمانی بود .منبع اصلی جمع آوری داده‌صها، مصاحبه‌صهای نیمه ساختارمند بودند .داده‌صها بر اساس رویکرد کوربین و اشتروس (2008) مورد تجزیه و تحلیل قرار گرفتند .یافته ها :هفت طبقه/درونمایه اصلی شامل تهدید ثبات خانوادگی، تکاپو برای درمان، ویژه یاری در فعالیت های روزمره زندگی، اداره مستمر بیمار، راهبری خانوادگی، تاب آوری/حفظ ثبات نسبی خانواده و دگرگونی های خانوادگی بودند از داده‌صها استخراج شد که ناظر بر زمینه بودند .طبقه حفظ ثبات خانوادگی به عنوان درونمایه اصلی پژوهش بود که همه‌صی طبقات دیگر به آن مربوط بودند .بحث و نتیجه گیری :شناخت مراحل تطابق خانواده با فعالیت های روزمره زندگی سالمند مبتلا به استروک و درک تجربیات اعضای خانواده، می تواند به توانمندی آنها در جهت رسیدگی صحیح تر به بیمار وکسب تطابق مثبت با مسائل آن کمک کند .نتایج این پژوهش می تواند دیدگاه عمیقی را برای پرستاران، پزشکان، متخصصان توانبخشی و طب سالمندی برای کمک به خانواده‌صها برای عبور از نقاط بحرانی تطابق خانواده از طریق ارایه کردن اطلاعات و آموزش های لازم، فراهم آورد، به طوری که در نهایت ثبات از دست رفته خانوادگی باز گردد و خانواده بتواند در برابر مشکلات رسیدگی به بیمار تاب بیاورد .پژوهش های آینده می تواند بر ارتقاء سطح تطابق خانواده های دارای بیماران سالمند مبتلا به استروک با تاکید بر ارتقاء ویژه یاری، اداره بیمار، راهبری خانوادگی و تاب آوری خانوادگی متمرکز گردد . کلمات کلیدی :تطابق خانواده، سازگاری، بیمار استروک، فعالیت های روزمره زندگی سالمند

Context: Stroke is viewed as family matter, because it causes physical sometimes irreversible disability. Family members play important roles in older patient's recover and rehabilitation. Given the increase of stroke incidence in the modern era and especially in the elderly people, there is not a clear explanation about family adaptation to the activities of daily living (ADL) of older people with stroke. Thus, this is study was done with the aim of describing the process of family adaptation to activities of daily living of older adults with stroke. Method of research: This qualitative research with using grounded theory methodology was performed in Sari and Behshahr cities in 2012-2014. The sampling method was initially purposeful then continued with theoretical sampling based on the collected data and emerging theory. The selecting of the participants continued until data saturation. 27 participants took part in this study consisted of: 19 family members who had and older people with stroke, 2 older patients with stroke, and 6 expert from health and treatment team. The main source of data collection were the semi-structured interviews. Data were analyzed by using Strauss and Corbin (2008) approach. Findings: Seven main categories/themes consisting of threatening of family stability, striving for treatment, handling of ADL, Continuous directing of patients, family management, resiliency/preserving family stability, and family change were explored from data which were considered in the context. The category of preserving family stability was identified as the main theme that all other categories were related to it. Discussion and conclusion: Identifying phases of family adaptation to ADL of older adult stroke and understanding of family members experience can help in the in the empowerment of these family in providing care to the patients and achieving positive adaptation. The result of this study can provide the nurses, physician and geriatric and rehabilitation experts with a deeper view to assist the families to pass the critical points of family adaptation through providing necessary information and education, so that the lost stability of the family return and family can endure the problems of patient's care Keywords: family adaptation, adjustment, stroke patients, elderly activities of daily living
