SK Jindal (pulmonary medicine, Postgraduate Institute of Medical Education and Research, India) served as editor-in-chief and, with the help of associate editors and sections editors, brought together the work of approximately 100 contributors--many based in India but a large number from the US, and a few from elsewhere in the world.
