BACKGROUND: Sleep quality is vital for proper functioning and proper execution of dailyactivities. Sleep quality is an important determinant of quality of life, not only poor sleepquality is associated with detrimental effects on health and hence poor quality of life butalso mortality risk in hemodialysis patients (1) In this study we will evaluate the associationbetween the sleep quality and bioimpedance indicators in dialysis patients at Bahrloohospital and Imam Khomeini hospital.Prevalence of sleep quality in End Stage Renal Disease patients (ESRD) is generally high,around 94.6% of ESRD patients had poor sleep quality a month before the intervention andafter some interventions the condition of poor sleep quality was improved to an about 82%.(4). Body hydration is a challenge to manage in dialysis patients and the dry weightadjustment to reach an appropriate dry weight in the subclinical hypervolemic patientsusing Bioimpedance technique resulted into improving sleep quality in patients who weresubjectively having poor sleep quality (7).Various methods exist in the clinical setting toevaluate the hydration status, bioimpedance analysis is among the methods used toevaluate hydration status in addition to other parameters of the body composition.7 Bioimpedance analysis (BIA) is a non-invasive technique which analyze body composition,it’s easy to use and rapid to obtain results with high safety to use (8), which has been usedwidely in clinical settings and other settings such as research centers which principally usesthe electrical properties (9), such as conductance and resistance of the body tissues such asbody water, muscle and others to measure different parameters in the body such as Totalbody water (TBW), Extracellular water (ECW), Intracellular water (ICW), Free Fat Mass(FFM), Body Fat Mass (BFM), Body Cell Mass (BCM), Soft Lean Mass (SLM), Percent Body Fat(PBF), Skeletal Muscle Mass (SMM), Body Mass Index (BMI) of the body composition bysupplying electrical current with different frequencies which flows through the body and theelectrical parameters ultimately will be measured. Therefore, these body parameters arebasically the bioimpedance indicators. For that reason, not only hydration status as anindicator of bioimpedance is an important element to evaluate in dialysis patients but alsoother indicators of bioimpedance BFM, FFM, BCM, PBF, SMM and BMI are also necessary toevaluate as they determine the general health in the population and so their effect on thesleep quality. Therefore, the main objective of this study is to assess the sleep quality andthe association of various bioimpedance indicators and the sleep quality in dialysis patientsusing bioimpedance analysis (BIA) technique.METHOD: The sleep quality was assessed by using the Persian version of Pittsburgh SleepQuality Index (PSQI-P). The bioimpedance indicators were obtained by using bioimpedancetechnology in which routinely it is done once in three months for each patient in dialysisunit. In this study the body composition analyzer InBodys10 will be used to collect thesebioimpedance indicators. In this study the last record of bioimpedance indicators of allparticipants recorded by the instrument will be collected and analyzed. Laboratory datawere obtained from the patient’s hospital files.8RESULTS: All data analyses were done by SPSS software, and all p-values were 2-tailed andp-value ≤0.05 will be considered significant. Mean and frequencies were reported. Among113 patients enrolled in this study we observed poor sleep quality in large proportion(53.98%) of patients and small proportion (46.02%) of patients had good sleep quality. weobserved significant association between sleep quality and mean gender (p=0.005), meanParathyroid hormone (p=0.025), mean dialysis shift (p=0.033), mean sleep drugsconsumption (p<0.001), mean Hypertension (p=0.002), ratio of ECW/TBW (p=0.039) andratio of ECW/ICW (p=0.041). Meanwhile no significant association between sleep qualityand mean of individual bioimpedance indicators including ECW, ICW, SMM, PBF and phaseangle. No significant association between sleep quality and mean age, mean BMI, mean ofother lab data as well as other factors including mean of dialysis route, mean of smoking,man of pack-year and mean of diabetes.CONCLUSION: sleep quality is poor in dialysis patients. Sleep quality is associated withgender, dialysis shift, sleep drugs consumption, parathyroid hormone, hypertension, ratio ofextracellular water to total body water and the ratio of extracellular water to intracellularwater. Sleep quality is not associated with age, BMI, extracellular water, intracellular water,skeletal muscle mass, percent body water, whole body phase angle, dialysis route, smoking,pack-year, hemoglobin, ferritin, TIBC, calcium, phosphorus, albumin, uric acid, kt/v (qualityof dialysis), onset of dialysis and diabetes.
