مقدمه :مشکلات شناختی یکی از علائم شایع در اماس است که درصد زیادی از افراد مبتلا آن را تجربه میکنند .اختلالات سرعت پردازش اطلاعات در میان مهم ترین و آسیب پذیرترین توانایی شناختی این افراد گزارش شده که روی سایر عملکردهای شناختی و عملکرد کاری( تحرک و تعادل) افراد با بیماری اماس تاثیرگذار است .هدف این پژوهش بررسی تاثیر توانبخشی شناختی مبتنی بر سرعت پردازش اطلاعات بر عملکرد کاری افراد با اماس بود .روش :در این مطالعه که از نوع مداخلهای نیمه تجربی و کارآزمایی بالینی است، شرکت کنندگان به صورت در دسترس انتخاب شدند .۳۶ بیمار با اماس به روش تصادفی بر حسب روز مراجعه در گروه کنترل و ۳۵ بیمار در گروه مداخله قرار گرفتند .مداخله که شامل تمرینات شناختی و تمرینات رایج کاردرمانی بود، طی ۱۲ جلسه ۴۵-۶۰ دقیقهای به صورت ۳ روز در هفته و برای مدت یک ماه در بیمارستان رفیده انجام شد .در گروه کنترل تمرینات رایج کاردرمانی انجام شد .متغیرها پیش از شروع مداخله، بعد از مداخله و دو ماه بعد از اتمام مداخله توسط تست ارزیابی سیستمهای تعادلی)BEST) ، آزمون جمع گام به گام شنیداری اعداد متوالی (PASAT) و آزمون نماد و رقم (SDMT) اندازه گیری شد .در نهایت تمام اطلاعات توسط نرم افزار SPSS ورژن ۲۳ تحت تجزیه و تحلیل آماری قرار گرفت .در مطالعه حاضر برای بررسی معناداری تفاوت بین نمرات در دو گروه مداخله و کنترل در طی زمان از آنالیز واریانس با اندازه گیری مکرر استفاده شد .یافتهها :نتایج نشان داد میانگین نمرات پیش آزمون، پس آزمون و پیگیری تستهای ذکر شده در دو گروه تفاوت معنادار داشت( ۰/۰۰۱ .)>pیعنی نمرات تستهایPASAT ، SDMT و BEST در گروه مداخله افزایش یافته و سرعت پردازش اطلاعات و عملکرد کاری افراد در این گروه بهبود قابل توجهی داشته است .نتیجهگیری :به نظر میرسد اضافه کردن توانبخشی شناختی مبتنی بر سرعت پردازش اطلاعات در کنار تمرینات روتین کاردرمانی به صورت مداوم و طولانی مدت میتواند موجب بهبود عملکرد کاری افراد با اماس شود .کلید واژهها :اماس، سرعت پردازش اطلاعات، عملکرد کاری، توانبخشی شناختی
Introduction: cognitive problems are one of the most prevalent symptoms in MS which affects a large percentage of affected people. Information processing speed disorders are reported among the most important and vulnerable cognitive ability of these individuals, which affects other cognitive functions and occupational performance (mobility and balance). The aim of this study was to investigate the effect of cognitive rehabilitation based on information processing speed on occupational performance of individuals with MS. Method: In this experimental and clinical trial study, participants were selected as available. 36 patients with MS were placed in control group and 35 patients in intervention group. The intervention included 12 sessions of 45-60 minutes performed 3 days a week for one month. The variables were measured before the began intervention, after intervention and 2 months after the intervention by BEST, PASAT and SDMT. Finally, all data analyzed by SPSS software. In this study, ANOVA with Repeated Measurements were used to analyze the significant difference between the scores in intervention and control group. Results: The mean scores of pretest, post-test and follow-up tests were significantly different in the two group (p<0.001). This means that the PASAT, SDMT and BEST tests scores increased over time in intervention group and the information processing speed and occupational performance has improved significantly. Conclusion: It seems that adding regular and long-term cognitive rehabilitation based on information processing speed along with routine occupational therapy exercises can improve the occupational performance of people with MS.
