مقدمه :تأمین ایمنی بیمار یکی از عوامل ارائه مراقبت باکیفیت از بیمار در بخش‌های روان‌پزشکی و موضوعی پر چالش برای مدیران و کارکنان این بخش‌ها است .هدف از انجام این مطالعه تبیین فرآیند و طراحی مدل تأمین ایمنی بیمار در مراکز روان‌پزشکی می‌باشد .روش :این مطالعه با رویکرد کیفی و با تمرکز بر نظریه زمینه‌ای انجام‌شده است .گردآوری داده‌ها به‌صورت هدفمند و با مشارکت ۲۶ نفر از کارکنان باتجربه، بیماران و خانواده بیماران از بیمارستان‌های روان‌پزشکی رازی، روزبه و آتیه در تهران و شفای رشت انجام‌شده است .روش‌های گردآوری داده‌ها شامل مصاحبه نیمه ساختارمند، مشاهده مستندات، یادداشت در عرصه و مصاحبه گروهی است .تحلیل داده‌ها با رویکرد کوربین و اشتراوس نسخه ۲۰۰۸ انجام‌گرفته است، بعد از ظهور متغیر مرکزی و ارائه نظریه، در قسمت دوم مطالعه مدل تأمین ایمنی بیمار با رویکرد واکر واوانت ۲۰۱۱ طراحی شد .یافته‌ها :تحلیل داده‌ها نشان داد که مهم‌ترین دغدغه مشارکت‌کنندگان" نگرانی از آسیب و خطرسازی بیماران "است .عوامل زمینه‌ای ایجادکننده این دغدغه "ناکارآمدی سازمان"،"جوکاری نامطلوب "و" سرچشمه‌های خطر" می‌باشند .مشارکت‌کنندگان برای مواجهه با دغدغه اصلی از راهبردهای" ارتقاء فرهنگ ایمنی" ،"مراقبت مقتدرانه " و" محافظت مسئولانه " استفاده می‌نمایند که پیامد " ایمنی مرزی " را به دنبال دارد .از یکپارچه‌سازی و ادغام مفاهیم به‌دست‌آمده مفهوم مرکزی " تدابیر هوشمندانه " به دست آمد که سایر طبقات را می‌توان با آن توضیح داد .در مرحله دوم مطالعه با استفاده از مفهوم مرکزی و مفاهیم به‌دست‌آمده از قسمت اول مطالعه( مطالعه کیفی) و بررسی مطالعات و مدل‌های مرتبط مدل" تدابیر هوشمندانه در تأمین ایمنی بیمار " در بخش‌های روان‌پزشکی طراحی گردید .نتیجه‌گیری :با توجه به یافته‌های این تحقیق توصیه می‌گردد عوامل زمینه‌ای مشخص‌شده موردتوجه قرارگرفته و راهبردهای ارائه‌شده در مدل شامل" مراقبت مقتدرانه" ،" محافظت مسئولانه " و" ارتقاء فرهنگ ایمنی " و" عوامل سازمانی و مدیریتی " در راستای تأمین ایمنی بیمار در بخش‌های روان‌پزشکی مورداستفاده قرار گیرند .کلمات کلیدی :ایمنی، پرستاران، روان‌پزشکی، بیمار، بیمارستان
Introduction: Providing patient safety is one of the factors behind the quality of patient care in psychiatric centers and a challenging topic for managers and staff in these centers. The purpose of this study is explaining the process and designing a model of patient safety providing in Psychiatric Centers. Methods: This study was conducted with a qualitative approach and focus on grounded theory. Data collection was done purposefully and with the participation of 26 experienced staff, patients and their families from psychiatric hospitals of Razi, Roozbeh, Atieh, and Shafa of Rasht. Data collection methods included semi-structured interviews, documents, field notes, and focus group. Data analysis was performed with Corbin and Strauss 2008 version. After the emergence of the core variable and presentation of the theory, in the second part of the study, the patient safety model was designed with Walker and Avant 2011 approach. Result: Data analysis showed that the main concern of the participants is "Concerns about Patients' Harm and Risk". The context behind this concern includes "inefficiency of the organization", undesirable work environment and "sources of danger". Participants used promote the culture of safety ", "powerful care" and "responsible protection" strategies to address the main concern, which results in "border safety". From the integration of the concepts, the central concept of "inteligence measures" was obtained. In the second stage of the study, first, the core variable was determined, then, using the concepts derived from the first part of the study (qualitative study) and the related studies and models, a model entitled inteligence measures" was designed. Conclusion: According to the findings of this research, it is recommended that the underlying factors identified are considered and the strategies presented in the model include "Powerful care", "responsible protection" and "promotion of safety culture" and "organizational and managerial factors" to be used in Providing patient safety in psychiatric centers.
