هدف :به نظر می‌صرسد بی‌صحرکتی ناشی از اسپلینت موجب کاهش حجم عضلات تنار و در نتیجه کاهش عملکرد دست می‌صگردد .هدف این مطالعه ارزیابی قطر، سطح مقطع، عملکرد و درد قبل و بعد از استفاده از اسپلینت در بیماران درجه 1 و 2 آرتروز مفصل کارپومتاکارپ اول و مقایسه آنها با گروه کنترل می‌صباشد .روش بررسی :در این مطالعه که از نوع قبل و بعد می‌صباشد 25 بیمار مورد ارزیابی به دو گروه استفاده کننده از اسپلینت و گروه مداخله تقسیم گردیدند .همه پارامترهای ذکر شده در بالا در روز اول و چهار هفته بعد از پیگیری مورد ارزیابی قرار گرفت .ابزار مقیاس بصری درد، پرسشنامه میشیگان دست و سونوگرافی به ترتیب برای اندازه‌صگیری درد، عملکرد، قطر و سطح مقطع عضلات مورد استفاده قرار گرفتند .یافته‌صها :استفاده از اسپلینت در گروه مداخله، کاهش درد را بعد از چهار هفته نشان داد این در حالی بود که هیچ کاهش قابل ملاحظه‌صای در پارامتر درد در گروه کنترل بعد از چهار هفته مشاهده نگردید .تغییرات عملکرد تقریبا در دو گروه مداخله و کنترل یکسان بود .استفاده از اسپلینت در این بیماران تاثیر چشمگیری بر کاهش قطر و سطح مقطع عضلات بعد از چهار هفته پیگیری نداشت، اگر چه تغییرات چشمگیری در کاهش قطر عضله متقابل کننده شست در گروه کنترل مشاهده گردید .نتیجه‌صگیری :استفاده از اسپیلنت تاثیر چشمگیری بر روی کاهش قطر و سطح مقطع عضلات بعد از چهار هفته پیگیری نداشت، اگرچه تفاوت معناداری در عضله متقابل کننده شست در گروه کنترل دیده شد .کلمات کلیدی :ارتوز سفارشی ساخت، استئوآرتریت مفصل کارپومتاکارپ اول، عملکرد، درد

Background: It was seemed that based on immobilization of thumb in using splints in patients with the first CMC joint OA, atrophy of thenar muscles can be happen.Objectives: the aim of this study was to evaluate the muscle diameter and cross sectional area, function and pain before and after given splint in patients with grade 1 and 2 thumb carpometacarpal joint OA compared to control group.Study design: Quesi experimentalMethod: Twenty five volunteer patients participated in this study. Referred patients divided in the splint group and control group. All above mentioned parameters were evaluated at baseline, and also 4 weeks in two groups. A visual analog scale, Michigan hand questioner, an ultrasound were used to measure pain, function, muscles diameters and cross section area.Results: Using the custom-made thumb splint demonstrated reduction in pain after4 weeks splint using. There was no significant difference on decrease of pain in control group after 4 weeks following period. Chang in function according to MHQ was same in two splint and control groups. Using splint has no significant effect on reduction of muscles diameter and cross section area (CSA) after 4 weeks split wearing, although there was significant difference only in Opponens pollicis of patients in control group.Conclusions: the splint using demonstrated no significant effect on reduction of muscles diameter and CSA after 4 weeks splint wearing, although there was significant difference only in Opponens pollicis of patients in control group after following time.Key words: custom-made orthosis, the first CMC joint OA, grip strength, pinch strength, hand functio
