ماستکتومی به عنوان یکی از روش‌های درمان سرطان پستان میتواند رضایتمندی از زندگی زناشویی و کارکرد خانوادة بسیاری از زوج‌ها را دچار اختلال کند .در این میان اضطراب‌های وجودی برخاسته از این رویداد بالقوه آسیب‌زننده و پاسخ‌های ناکارآمد به آن ممکن است نقش میانجی‌گری داشته باشد .هدف از پژوهش حاضر بررسی اثربخشی زوج‌درمانی مبتنی بر تلفیق رویکرد وجود گرایی با مدل السون بر رضایتمندی زناشویی و کارکرد خانواده زنان ماستکتومی شده و همسرانشان است .در این پژوهش از طرح مطالعه تجربی تک آزمودنی از نوعA - Bاستفاده شده است .جمعیت آماری پژوهش حاضر را زوج‌های مراجعه‌کننده به بخش پرتودرمانی بیمارستان امام حسین) ع (شهر تهران، تشکیل می‌دهند که از میان آن‌ها سه زوج که میانگین سنی‌شان 20 تا 50 سال بوده، همسرانشان تحت عمل جراحی ماستکتومی) تخلیه پستان (قرارگرفته بودند، غدد سرطانی به سایر نقاط بدن سرایت نکرده و سابقه بیماری روان‌پزشکی قبل از ابتلا به سرطان را نداشتند، از طریق روش نمونه‌گیری مبتنی بر هدف انتخاب‌شده و مداخله در قالب 12 جلسه 90 دقیقه‌ای هفته‌ای یک بار، متناسب با نیازهای خاص آن‌ها طراحی و اجرا گردید .میزان رضایتمندی زناشویی و کارکرد خانواده زوج با استفاده از مقیاس سازگاری زناشویی اسپانیر (DAS) و ابزار سنجش کارکرد خانواده (FAD) بررسی گردید .مقایسه نمودار روند نمرات زوج در طی 9 بار تکمیل پرسشنامه) سه خط پایه، چهار اندازه‌گیری در حین اجرا و دو سنجش پیگیری (و محاسبة درصد بهبودی، نشانگر افزایش نمره در مقیاس سازگاری زناشویی و کارکرد خانواده بود .لذا به نظر می‌رسد این زوج‌درمانی تلفیقی توانسته است با مدنظر قرار دادن اضطراب‌های وجودی زوج، رضایتمندی زناشویی و کارکرد خانواده این دسته از زوج‌ها را ارتقا دهد .کلیدواژه‌ها :رضایتمندی زناشویی، کارکرد خانواده، زوج‌درمانی تلفیقی وجود گرایی با مدل السون، سرطان پستان، ماستکتومی

Mastectomy as a treatment for breast cancer can disturb marital satisfaction and family functioning of many couples. In this way, existential anxiety stemming from this potentially deleterious event and inefficient responses to them, could be mediating. The purpose of this study is to investigate the effectiveness of an eclectic method to couple therapy based on existential approach and Olson's marital enrichment model on marital satisfaction and family functioning of women who had undergone mastectomy and their husbands. In this study, a single subject research design (Type A-B) is used. The study population comprised couples who had referred to Radiotherapy department of Imam Hussein hospital in Tehran, that among them three couples whose average age was 20 to 50 year, wife's had undergone mastectomy, tumor has not spread to other parts of the body, and had no prior history of psychiatric disorders before cancer, were selected through purposive sampling and Intervention in 12 sessions of 90 minutes once a week, has been designed to suit their specific needs. The level of couples marital satisfaction and family functioning was evaluated using Dyadic Adjustment Scale (DAS) and Family Assessment Device (FAD). Comparing couple's scores on the diagram during 9 time measurement (3 time baseline, 4 times during intervention, and 2 times follow up assessment) and calculating recovery percent, represent increasing in score of marital adjustment and family functioning scale. So it seems that, this kind of an eclectic couple therapy, by considering couples existential anxiety, has been promoted marital satisfaction and family functioning of these couples Keywords: Marital satisfaction, Family functioning, eclectic couple therapy based on existential approach with Olson's model, Breast cancer, Mastectomy
