This survey results can help to manager to recognize weakness points for applying the quality Management system.Key words: Total Quality Management (TQM), gap, conception (present condition),expectation (optimum condition), fuzzy approach, Rehabilitation Centers بwelfare organizations, and analyzed this gap with fuzzy approach, in 2011.Method: This was a descriptive-analactic, cross sectional survey. Sampling was conducted by census approach. All staffs of Gorgan private welfare centers were 320 people. sampels number were based on objects.With exclude criteria 180 Person has selected, and after include criteria 81 people were remained as the sample. gap between conception and expectation of Total Quality Management factors were assessed by TQM questionnaire. It has used to similar study in Iran. The questionnaire was assessed 8 factors of TQM: Management commitment, Empowerment, Quality structure, Compensation system,Management style, Team work, Quality culture and Customer oriented. Validity of the questionnaire was verified with Lawshe's techniqueand the Reliability with (..= 0.932) was verified. SPSS, EXCEL, Spearman, Pearson Chi-Square, and fuzzy wilcoxen-Mann Whitney approach were used for statistical analysis.Result: Findings showed total fuzzy average of conception were (0.59, 0.71, 0.83) and total fuzzy average of expectation were (0.822, 0.923, 0.982). Total fuzzy average of expectation was more than total fuzzy average of conception. Compensation system Factor with fuzzyaverage of conception(0.45, 0.58, 0.72) and with fuzzy average of expectation (0.821, 0.923,0.981) showed the most gaps and Management commitment Factor with fuzzy average of conception (0.62, 0.74, 0.86) and with fuzzy average of expectation(0.814, 0.915, 0.976)showed the least gap.Conclusion: Results showed that there was significant gap between all factors of total quality management.
