Predicting soft tissue release for balancing in total knee arthroplasty based on angular parameters in alignment view with a mechanical concept in Emam Khomani hospital 2018-2020

Introduction: According to good final result in total knee arthroplasty, this operation is increasing in the world. For earning best result, it is necessary to select suitable patient and optimize the medical situation. During operation in addition to select best prosthesis size, surgeon should maintain the balance between the extension and the flexion gap of the knee. However, amount of soft tissue releasing prediction depends on the pre operation planning which has been made the surgeons more oriented in preforming the surgery and it helped them to minimize the surgery timesMethod: Method: the studied cases are documentation of non-traumatic and non-hemophilic patients between 2018 and 2020 that have undergone total knee replacement surgeries, with revision or conversion surgeries as exclusion criteria. The three joint imaging was taken from the medical decimation and records that were archived in the Emam Khomeini hospital archive.Result: out of 1260 patients 1218(96.7%) were varus while 42(3.3%) valgus, the range in varus knees (0º-48º±6.984) and in valgus knees from (-1) to (-26) with SD(±6.742).in mild varus knees (<10º ) the release stage of 1,2,3(93.2%,3.8%,3%) , in moderate varus knees (10º -20º) the release stage of 1,2,3(69.2%,27.8%,3%) and in sever varus knees (>20º) the release stage of 1,2,3(68.5%,27.3%,4.2%).the mean of JCA/VA in release stage 1,2 and 3 (0.764±0.619827,0.390±0.090940, 0.148±0.124002) (p<0.001).In varus knees 89.2% of stage release 1 JCA/VA (≥0.51) 94.5% of stage release 2 JCA/VA (0.31-0.50) and 100% of stage release 3 JCA/VA (≤0.30) while in valgus knees 54.2% of stage release 1 JCA/VA (≥0.51) 33.3% of stage release 2 JCA/VA (0.31-0.50) and 100% of stage release 3 JCA/VA (≤0.30)Conclusion: We can predict pre-operative soft tissue release based on JCA/VA if it is ≥ 0.51,0.31-0.50 and ≤ 0.30 the release stage will be 1,2 and 3 respectively, increasing VA doesn’t indicate increasing of soft tissue release.
