Background: In total knee arthroplasty, the position and angles of components cuts are estimated in the preoperative planning base on three joint alignment view x-rays in coronal planes however several concerns about axial rotation of components still exist. As they would need to be precisely measured by pre-op CT-scan or MRI which are not safe nor costly effective respectively or based on intra-operative axes of previously described PCA, TEA or APA lines. Thus, the need for preoperative safe and easy estimation of rotational cuts made us study the relation between three joint parameters and degree of femoral external rotation cut in order to have proper femoral component rotationMethod this study was designed as retrospective cohort on 1260 Patients undergone TKA during 2018-2020 at Emam Khomeini hospital, Tehran, Iran. Revision and conversion surgeries were excluded. Three joint alignment parameters and the extent of external rotation cut based on documented intraoperative measures were derived from patient’s files and analyzed by SPSS version 26. Result: The femoral component had significantly more external rotation in the female group than in the male group (3.05±0.425 vs.3.41±0.932, p-value=<0.001). the mean age was more in male than female (69.44±8.24 vs. 64.80±7.79, p-value=<0.001). LDFA and MPTA have a statically significant inverse proportion relationship with external rotation (0º,3º,5ºand 7º) and its (p= 0.041,0.040) respectively, JCLA has significant direct relationship with external rotation(0º,3º,5ºand 7º) (p<0.001),while MMA has no significant relationship with external rotation(0º,3º,5ºand 7º) (p=0.483).JCA/LDFA has statically significant with external rotation(0º,3º,5ºand 7º) (p<0.001), based on ROC curve the cutoff point is 0.1105 with sensitivity 0.671& specificity 0.663.JCA/MPTA has statically significant with external rotation(0º,3º,5ºand 7º) (p<0.001), based on ROC curve the cutoff point is 0.1296 with sensitivity 0.590& specificity 0.76,1but JCA/VA has no significant relationship with external rotation (p=0.124).Conclusion: MPTA and LDFA have inverse proportion but JCLA direct proportion to femoral external rotation, females have chance to has more external rotation than males.
